摘要
目的探讨磁敏感加权成像(susceptibility weighted imaging,SWI)显示颅内肿瘤内部结构的能力。资料与方法对21例颅内肿瘤患者行常规MR[T2WI、液体衰减反转恢复序列(FLAIR)、T1WI、T1WI+增强T1WI]和SWI,其中7例行时间飞跃法-磁共振血管成像(TOF-MRA),5例于增强前后行SWI。将所有图像传至工作站详细记录肿瘤内部结构,SWI图像后处理采用最小密度投影(MinIP)获得。并将SWI图像与病理结果进行对照分析。结果21例共23个肿瘤病灶。SWI对肿瘤内出血灶检出率为91.3%,常规MR序列为34.8%,两者之间差异具有统计学意义(P=0.002,P<0.05)。在8个肿瘤病灶中发现了18个钙化灶;SWI对肿瘤坏死及实质部分的显示也与常规序列接近;SWI在22个病灶中显示了肿瘤相关静脉,常规MR序列未能显示,7例中MRA显示2例肿瘤内部血管;增强后SWI能显示的细小静脉结构更多,对出血灶和小静脉显示更加清晰;SWI对肿瘤内部结构描述与病理结果基本一致。结论SWI在显示细小出血灶、肿瘤相关静脉结构等方面优于常规MR序列,是研究肿瘤内部结构的一种新的方法。
Objective To evaluate the value of susceptibility weighted imaging (SWI) in demonstrating the internal architecture of brain tumors. Materials and Methods A total of 21 patients with brain tumors received conventional sequences (T1WI, T1WI postcontrast, T2WI, fluid-attenuated inversion recovery and SWI), among them, 7 patients received TOF-MRA and 5 with pre and post contrast SWI. Primitive data were processed by the AW4.1, GE medical systems. Min-IP images were generated to demonstrate the complete morphology of veins which were compared with pathological results. Results 23 tumors were found in 21 patients. In the evaluation of blood products, the positive rate of SWI (91.3%) was significantly higher than that of conventional MR( 34.8% ) ( P 〈 0.05 ) ; among the 23 tumors, 18 calcifications were found ; SWI also could show necrosis and substantial part of tumor similiar to conventional sequences ; SWI displayed vasculature of tumors in 22 cases, which were not found by conventional sequences, among 7 cases, vasculature of tumors were found in 2 cases by MRA ; postcontrast SWI was better than non-contrast SWI in visualizing blood products and could show more veins; there was close correlation between pathological results and SWI in depicting internal architecture of lesions. Conclusion SWI offers a new way to show the internal chitecture of brain tumors, it is proved to be more useful than conventional MRI in showing blood products and details of tumor-related veins.
