摘要
目的探讨阻塞型睡眠呼吸暂停低通气综合(OSAHS)合并脂肪性肝病发病的危险因素。方法选择进行肝脏B超检查的住院阻塞型睡眠呼吸暂停低通气综合征患者102例。根据超声影像的诊断结果分为阻塞型睡眠呼吸暂停低通气综合征合并脂肪性肝病组(60例)和阻塞型睡眠呼吸暂停低通气综合征未合并脂肪性肝病组(42例),分析患者体重指数(BMI)、收缩压(SBP)、舒张压(DBP)、空腹血糖(FBG)、总胆固醇(TC)、甘油三酯(TG)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)、丙氨酸氨基转移酶(ALT)、天冬氨酸氨基转移酶(AST)、γ-谷氨酰转肽酶(GGT)、睡眠呼吸暂停低通气指数(AHI)与脂肪性肝病之间的关系。结果OSAHS合并脂肪性肝病组的BMI、SBP、DBP、FBG、ALT、AHI高于对照组(P均<0.05),空腹血糖升高、肥胖、高血压、AHI均是患者脂肪性肝病形成的独立危险因素。结论空腹血糖升高、肥胖、收缩压高、睡眠呼吸暂停低通气指数是OSAHS患者脂肪性肝病发病的独立危险因素;慢性间歇缺氧可能通过加剧胰岛素抵抗等途径促进脂肪性肝病的形成。
Objective To analyze the risk factors in patients with obstructive sleep apnea hypopnea syndrome (OSAHS) combined with fatty liver disease. Methods According to the diagnosis of B ultrasound, a total of 102 inpatients with OSAHS were recruited, and divided into two groups, OSAHS combined with (60 cases) and without (42 cases) fatty liver disease group. The con'elation between Fatty liver disease and these indices was analyzed, including Body mass index (BMI), systolic blood pressure (SBP), diastolic blood pressure (DBP), total cholesterol (TG), triglyceride (TG), high density lipoprotein cholesterol (HDL-C), low density lipoprotein cholesterol (LDL-C), aspartate aminotransferase (AST), alanine aminotransferase (ALT), gamma-glutamyl-transpeptidase (GGT) and apnea hypopnea index (AHI). Results (1) The levels of FBG, BMI, SBP, DBP, ALT, AHI in OSAHS combined with fatty liver disease group were significantly higher than those in control group (P〈0.05). (2) The results of logistic regression analysis showed that increasing level of fasting blood glucose, obesity, high SBP and AHI were independent risk factors for patients with OSAHS combined with fatty liver disease. Conclusions The independent risk factors for patients with OSAHS combined with fatty liver disease were increasing level of FBG, obesity, high SBP and AHI. Chronic intermittent hypoxia may produce fatty liver disease through intensifying insulin resistance.
