周围神经刺激器定位在臂丛神经阻滞中的应用

Application of Peripheral Nerve Stimulator During Brachial Plexus Block

摘要 目的:比较周围神经刺激器定位与异感定位用于臂丛神经阻滞的效果。方法:选择拟行上肢手术的患者60例,ASAI~II级,随机分为两组,每组30例:周围神经刺激器定位组(A组)、采用传统异感定位组(B组)。两组均给予1.6%的利多卡因25ml。记录定位时间、起效时间,评定手术全程的阻滞效果。结果:A组定位时间4.2±2.6分钟,明显短于B组的10.1±4.7分钟(P<0.01)。A组起效时间10.6±2.2分钟,明显快于B组13.2±3.4分钟(P<0.05)。A组阻滞效果优良率为93.3%,明显高于B组80%(P<0.05)。结论:周围神经刺激器定位行臂丛神经阻滞定位时间短,阻滞起效快,阻滞效果好。 Objective: To compare the efficacy of brachial plexus block guided by peripheral nerve stimulator with that guided by paresthesia technique. Methods: 60 ASA class I or I1 patiens scheduled for upper extremity operation were randomly devided into two groups, 30 cases each, in group A peripheral nerve stimulator was used to locate the brachial plexus and in group B block was performed in the traditional way based on paresthesia. 25ml 1.6% lidocaine was administered in both groups. The identification time for the target nerves and the onset time were recorded. The anesthetic effect was assessed. Results: The identification time in group A was 4. 24-2. 6min, which was significantly shorter than that in group B 10. 1-1-4. 7rain(p〈0. 01). The onset time in group A was 10. 6-1-2. 2rain, which was significantly faster than that in group B 13. 2-4-3. 4min(p〈0. 05). The effective rate of anesthetic effect in group A was 93. 3%, which was significantly greater than that in group B 80^( p〈0. 05). Conclusion: Brachial plexus block guided by peripheral nerve stimulator may shorten the identification time , onset time and provide excellent block.
