AD和MCI病人前额叶注意功能区fMRI检测 被引量:10

A BOLD-fMRI STUDY OF PREFRONTAL CORTEX IN ALZHEIMER DISEASE,MILD COGNITIVE IMPAIRMENT AND NORMAL CONTROL SUBJECTS

摘要 目的应用功能磁共振成像(fMRI)观察阿尔茨海默病(AD)、轻度认知功能损害(MCI)病人在汉语Stroop任务操作中的脑皮质功能区活动情况。方法选择AD病人11例,MCI病人10例,和按年龄、性别、文化层次进行匹配的正常对照组10例。fMRI运用刺激-休息-刺激的模式,刺激任务采用汉语Stroop任务。测定分析3组被试反应时间、漏报率、错误率的差别。采用AFNI软件将图像数据进行处理和分析,对3组脑激活图像的激活部位及范围进行比较。结果AD组完成汉语Stroop色词任务反应时间长于对照组,错误率、漏报率高于对照组(Z=-3.38^-2.82,P<0.01);MCI组反应时间长于对照组(Z=-2.04,P<0.05),漏报率、错误率与对照组无明显差异(Z=-1.14、-0.89,P>0.05)。MCI组在双侧背外侧前额叶(DLPFC)、双侧后顶叶激活体积明显大于对照组(Z=-2.663^-2.268,P<0.05);AD组在双侧DLPFC、腹内侧前额叶(VLPFC)激活体积明显小于对照组(Z=-3.029^-2.536,P<0.05),在双侧后顶叶与对照组比较无明显差异(Z=-0.423^-0.387,P>0.05)。结论AD以前额叶功能损害为主,MCI以前额叶和后顶叶功能代偿为主,提示老年性认知损害不同阶段具有不同的神经病理学特征。 Objective To investigate the prefrontal lobe activities in patients with Alzheimer disease (AD), mild cogni tire impairment (MCI) and normal controls during Chinese Stroop color-word task performance with fMRI. Methods Eleven AD patients, 10 MCI patients and 10 normal control subjects were enrolled. They were matched by age, sex and education background. All the participants underwent fMRI with the mode of stimulus-rest-stimulus when performing Chinese Stroop test. The reaction time, error rate and miss rate were measured and compared. The imaging data were analyzed and compared by AFNI software. Results The time spent for Stroop task in AD was longer, the error and miss rates were higher than those of the normal control (Z=-3.38--2.82,P〈0.01). The response time in MCI showed longer than that in the control and the differences were not significant between the two groups in terms of error rate and miss rate (Z=-2.04 ,P〈0.05). Compared with controls, MCI patients showed distinctly increased prefrontal cortex activation, including the right DLPFC (dosolateral prefrontal cortex) and bilateral inferior parietal lobe (Z=--2. 663--2. 268,P〈0.05), while AD patients exhibited decreased fMRI responses in the regions of bilateral DLPFC and VLPFC (ventralateral prefrontal cortex) (Z=-3. 029--2. 536, P〈0.05), no difference was noted in bilateral posterior apical lobes (Z=-0. 423--0. 387,P〈0.05). Conclusion In AD, the function lesion mainly locates in prefrontal; in MCI, mainly with functional compensation of prefrontal and posterior parietal cortex, which suggests that different neuropathological characteristics exist in different phases of dementia in the elderly.
