经阴道多普勒超声检测卵巢早衰患者卵巢血流变化与卵巢激素水平的相关性 被引量:15

The Relation between Ovarian Blood Index and Hormone Level in POF

摘要 目的探讨卵巢早衰患者卵巢血流指标与激素水平的相关性。方法研究对象为临床及实验室诊断为卵巢早衰的20例患者(POF组)和正常育龄妇女20例(CTR组)进行彩色多普勒超声检查,并检测两组同时期的血清雌二醇(17β-estradiol,E2)、卵泡刺激素(follicle-stimulating hormone,FSH)和促黄体生成素(luteinizing hormone,LH)。结果POF组FSH、LH、阻力指数(resistive index,RI)、搏动指数(pulsatilityindex,PI)值高于CTR组,E2、收缩期峰流速(peak systolic velocity,PSV)明显低于CTR组,两组间卵巢动脉血流指标、激素水平的差异均有统计学意义(P<0.05)。POF组卵巢血流PSV与E2呈正相关(r2=0.93,P<0.05),PSV与FSH呈负相关,(r2=-0.64、-0.55,P<0.05)。PI、RI与激素无相关性。结论卵巢早衰患者的卵巢动脉血流、激素水平与卵巢功能密切相关,经阴道超声检查卵巢血流变化结合卵巢激素水平为卵巢早衰的早期诊断和预测治疗提供了新的方法。 Objective To study the relation between ovarian blood index and hormone level in POF. Methods Electing 20 female patients suffered from POF through clinical and laboratory diagnosis as experimental object, 20 healthy females as control, 40 females all received color Doppler ultrasonography, The concentrations of three contemporaneous hormones (including 17β - estradiol, E2, follicle - stimulating hormone, FSH, luteinizing hormone) were measured by quantitative ELISA. Result The values of RI, PI, FSH、 LH in POF increased, compared with the values of control,while values of PSV, E2 decreased significantly. There are statistical significance between ovarian blood index and hormone level. The analysis of correlation Indicated the positive correlations existed between PSV and E2(r^2 =0. 93,P 〈0. 0.5), while negative correlations with FSH; there are no relationship PI、RI,LH and hormone. Conclusion ovarian artery bloodstream、hormone level and ovarian function exist closed correlation in POF, providing new means to search the etiopathogenisis and predicted therapy in the POF through ovarian blood index and hormone level.
