糖化白蛋白评价急性心肌梗死患者糖代谢水平的临床研究 被引量:9

摘要 目的:用糖化白蛋白(GA)评价急性心肌梗死(AMI)患者发病前2~3周血糖的平均水平,探索在发生AMI前是否已经存在糖代谢的异常。方法:选择无糖尿病AMI患者(AMI组)与无冠心病和糖尿病者作对照(对照组),采用酶法糖化白蛋白测定试剂检测血浆GA水平,AMI30d随访时行OGTT试验,比较入院与发病30dGA和血糖水平。结果:(1)AMI组患者GA水平较对照组高,差异有统计学意义(P<0.05)。(2)AMI组发病30d时空腹(FPG)、血糖餐后2h血糖(2hPG)水平较入院时降低,而GA值反而升高,差异有统计学意义(P<0.05),糖化血红蛋白(HbA1c)水平前后比较未见差异(P>0.05)。结论:(1)AMI时的高血糖可能是机体之前就存在的糖代谢紊乱,而不是单纯的应激反应。(2)GA比FPG、2hPG、HbA1c能更好地反映AMI患者短期内的糖代谢的状况。
