摘要
目的评价上海市嘉定区4所医院抗菌药管理和使用现状。方法采用回顾性方法依据国际抗微生物药物使用调研指标对嘉定区4所医院2008年住院病人进行抽样调研。医院整体情况数据通过管理部门和药库获得,医嘱行为和病人治疗情况通过系统抽样方法采集。结果4所医院共收集病例468份,住院期间使用抗菌药物≥1种的病人占所有收集病例的56.7%～80.0%;病人平均使用抗菌药物品种数分别为1.7±s1.1、1.9±0.8、2.1±0.9和2.5±1.0;抗菌药物治疗(非预防Ⅰ、Ⅱ类切口感染)的平均日数分别为5.0±2.6、8±4、8±4和6±3;接受抗菌药物治疗病人的平均住院日数分别为14±7、13±18、10±6和12±15;2家区级医院接受抗菌药物治疗者分别有10%和41%接受药敏试验。结论4所医疗机构住院病人中抗菌药物使用比例高于上海市地区样本医院的同期水平,应强化《抗菌药物临床应用指导原则》的实施以提高合理用药水平。
AIM To analyze the utilization of antimicrobial drugs in four hospitals of Jiading District of Shanghai . METHODS The selected indictors in hospitalized patients recommended by WHO/INRUD were applied by WHO principles on rational antimicrobial agent use to evaluate the use of antimicrobial agents. The health facility information was gained from hospital administration section. The prescription and treatment data were collected from the discharged patients＇ medical history. RESULTS Totally, 468 patients were collected from the four hospitals. The percentage of hospitalization with antimicrobials prescribed was 56.7% - 80.0%. The average number of antimicrobial agents prescribed per hospitalization was （1.7 ± s 1.1）, （1.9 ±0.8）, （2.1 ± 0.9）, and （2.5 ± 1.0）. The average duration of prescribed antimicrobial treatment was （5.0 ±2.6）, （8 ± 4）, （8 ± 4）, and （6 ± 3） d. The average hospital stay of patients who receive antimicrobial agent was （14 ± 7）, （ 13 ± 18）, （ 10± 6）, and （ 12± 15） d. The percentage of drug sensitivity test of two district hospital was 10% and 41%. CONCLUSION Compared with sample of hospital in Shanghai, it is high in antimicrobials prescribed in hospital patients. The implement of guidelines need to be improved and strengthened to promote rational use of drugs.
