真菌性角膜炎真菌鉴定及药物敏感实验分析 被引量:8

摘要 目的:分析真菌性角膜炎致病菌的种类和常用抗真菌药物敏感性。方法:分离鉴定54株真菌性角膜炎致病菌,采用NCCLS M38-A文件的实验条件进行常用抗真菌药物敏感试验。结果:54株真菌经培养证实均为丝状真菌,其中镰刀菌属37株(37/54)、曲霉菌属14株(14/54),未定型3株(3/54)。镰刀菌属中对二性霉素B敏感31株(31/37),对特比奈芬敏感27株(27/37),对酮康唑敏感23株(23/37),对伊曲康唑敏感24株(24/37),对咪康唑敏感23株(23/37),对5-氟胞嘧啶敏感18株(18/37)。曲霉菌属对二性霉素B敏感12株(12/14),对特比奈芬敏感11株(11/14),对酮康唑敏感10株(10/14),对伊曲康唑敏感10株(10/14),对咪康唑敏感9株(9/14),对5-氟胞嘧啶敏感7株(7/14)。全部54株真菌对二性霉素B敏感46株(46/54),对特比奈芬敏感40株(40/54),对酮康唑敏感35株(35/54),对伊曲康唑敏感36株(36/54),对咪康唑敏感34株(34/54),对5-氟胞嘧啶敏感28株(28/54)。结论:真菌性角膜炎以镰刀菌感染为主;二性霉素B敏感性较高,可作为真菌性角膜炎一线用药。 AIM: To study the pathogenic characteristics and antifungal susceptibility of fungal keratitis. METHODS: The pathogen of fungal keratitis was isolated and classificated. Antifungal susceptibility test in vitro (NCCLS, M38-A)were used to evaluate susceptibility (MIC) of these fungal pathogens against amphotericin B, terbinafine,tonconazole, itraconazole, miconazole and 5- flucytosine. RESULTS: Fifty-four cultures were positive. Fusarium species was the most frequent corneal fungal pathogen (37/54), followed by Aspergillus species (14/54). The sensitivity of Fusarium species to amphotericin B, terbinafine tonconazole, itraconazole, miconazole and 5-flucytosine was (31/37), (27/37), (23/37), (24/37), (23/37) and (18/37) respectively. The sensitivity of Aspergillus species to amphotericin B, terbinafine tonconazole, itraconazole, miconazole and 5-flucytosine was (12/14), ( 11/14 ), (10/14), ( 10/14 ), ( 9/14 ) and ( 7/14 ) respectively o The sensitivity of all species to amphotericin B, terbinafine, tonconazole, itraconazole, miconazole and 5-flucytosine was (46/54), (40/54), (35/54), (36/54), (34/54) and (28/54) respectively. CONCLUSION: Fusarium species may be the main corneal fungal pathogen. Amphotericin B seem to have a high antifungal activity in vitro and may be used as first medication for fungal keratitis.
