药物涂层材料对支架置入后血管内再狭窄的影响 被引量:1

Influence of drug-eluting materials on endovascular restenosis following stent implantation

摘要 目的:评价不同材质的血管内支架置入的安全性,分析不同血管内支架材料置入后抗凝血性、耐腐蚀性和生物相容性的变化。方法:以计算机检索方法在检索中国期刊全文数据库中(CNKI:2002/2007)检索关于不同血管内支架材料置入后抗凝血性、耐腐蚀性和生物相容性的变化的自身前后对照实验,检索词为"血管内支架、药物涂层材料、再狭窄"。检索后对每项研究的资料结果进行提取、分析。结果:共有19项实验2731例血管内支架置入患者符合纳入标准,血管内支架置入后血管脉狭窄率均较术前降低,随访结果显示药物涂层支架较金属裸支架能明显减低再狭窄率,支架形态良好,无变形,支架处血流通畅,无血管内再狭窄发生。同时对比得出血管内支架置入后的炎症过程的激活对血管内皮的增生与再狭窄起着重要作用。结论:血管内支架置入治疗动静脉狭窄的成功率高,具有很好的临床效果,特别是药物涂层支架能够显著降低血管内再狭窄的发生率,临床应用安全有效。但由于纳入试验少,证据的强度不足,其他有效性指标和安全性,有待更多证据加以评价。 OBJECTIVE: To evaluate the safety of different types of endovascular stent and investigate the changes in anticoagulation, corrosivity-resistance and biocompatibility following stent implantation. METHODS: Computer-based search of CNKI was performed to search self-controlled experiments regarding changes in anticoagulation, corrosivity-resistance and biocompatibility before and after stent implantation published between 2002 and 2007 with the keywords of endovascular stent, drug-eluting materials, restenosis. The data were extracted and analyzed. RESULTS: A total of 19 experiments involving 2 731 cases undergoing endovascular stent implantation were included in the review. The incidence rate of stenosis following stent implantation was reduced. The follow up results show that drug-eluting stent displayed reduced restenosis rate compared with metal bare stent, and the stent appearance was good with patent blood flow. In addition, the comparison suggests that the activation of inflammation process following stent implantation plays a key role in blood vessel endothelial hyperplasia and restenosis. CONCLUSION: Endovascular stent implantation has high success rate in treating arterial and venous stenosis, showing favorable clinical effect, in particular, the drug-eluting stent significantly reduces incidence of restenosis with efficacy and safety. However, there are limited experimental data and efficacy indexes, which need further evaluation.
