摘要
目的研究床边纤维支气管镜（简称纤支镜）技术在EICU人工气道管理中的疗效。方法对EICU开放人工气道的34例次的危重患者进行床旁纤维支气管镜检查和肺泡灌洗治疗，并对其痰标本阳性率、血气分析指标的变化以及胸片的变化进行前后自身对照。结果经纤支镜留取的痰标本的痰培养阳性率明显高于常规途径留取的痰标本培养，治疗前后血气分析中pH、PaO2、PaCO2及SaO2等各项指标，差异有统计学意义（P〈0．05）。结论床边纤维支气管镜技术在EICU人工气道开放的危重患者中，可以大大提高危重患者肺部感染的早期诊断率，值得作为一种早期治疗的常规治疗手段。
Objective To research on the curative effect of bedside fiberbronchoscope technology in EICU artificial air passage management. Method Bedside fiberbronchoscope examination and lung lavage were performed for 34 severe patients with artificial air passage in EICU ward, and positive rate of sputum sample, blood gas analysis index and chest X ray before and after the treatment were compared. Result The positive rate of sputum culture from the above mentioned method was obviously higher than that from routine method ; after the treatment, the indexes of pH, PaO2 , PaCO2 and SaO2 in the blood gas analysis were better markedly （ P 〈 0.05 ）. Conclusion The bedside fiberbronchoscope technology in the severe patients with open artifieial air passage in EICU can improve the early diagnosis of lung infection, and should be used as routine therapy.
