单侧眼眶骨折眼球突度的CT测量方法与可靠性评价 被引量:6

Measurement of the globe proptosis by CT and its accuracy in patients with unilateral orbital fracture

摘要 目的评价计算机辅助CT测量单侧眼眶骨折眼球突度的可靠性。方法2004年9月至2007年12月在北京大学口腔医学院创伤中心接受治疗的单侧眼眶骨折患者25例,术前、术后均拍摄眼眶CT轴位片。分别采用Hertel眼突计测量、以眶外缘为参照点CT测量法和以视神经孔为参照点的CT测量法,于术前、术后测量健侧眼球突度,并对测量结果进行比较,评价以上3种测量方法的可靠性。结果Hertel眼突计测量结果:术前为(13.9±3.1)mm,术后为(12.5±2.2)mm,两者测量差值为(2.14±1.1)mm;以眶外缘为参照点的CT测量结果:术前为(16.1±2.6)mm,术后为(15.7±2.3)mm,两者测量差值为(1.43±1.24)mm;以视神经孔为参照点的CT测量结果:术前为(45.3±4.8)mm,术后为(46±3.9)mm,两者测量差值为(2.29±2.13)mm。术前、后测量结果比较,只有Hertel测量的差异有统计学意义(P〈0.05)。结论在单侧眼眶骨折患者的眼球突度测量方面,根据CT数据,应用计算机软件以眶外缘为参照点进行眼球突度测量的方法较Hertel眼突计测量更加可靠。 Objective To evaluate the accuracy of measurement of the globe proptosis by CT in patients with unilateral orbital fracture. Methods 25 patients with unilateral orbital fracture were included. CT was performed before and after operation. The globe proptosis on the unaffected side was measured before and after operation by using Hertel exopbthalmometer(HE), CT images with orbital lateral rim(OLR) and the optic nerve foramen(ONF) as reference points. The accuracy was evaluated comparably by statistical analyses. Results The preoperative and postoperative average globe proptosis were (13.9 ± 3.1 ) mm and (12.5 ± 2.2) mm in HE group, ( 16.1 ±2.6) mm and ( 15.7 ± 2.3 ) mm in the OLR group, ( 45.3 ± 4.8) mm and(46 ± 3.9) mm in the ONF group, showing a significant difference between the pre-and post-operative results in HE group, but not in OLR and ONF groups. Conclusions CT measurement with the orbital lateral rim as reference point is more reliable than Hertel exophthalrnometer measurement for globe proptosis.
