低浓度罗哌卡因复合艾司洛尔及舒芬太尼用于颈丛阻滞的临床研究

摘要 目的观察低浓度罗哌卡因复合艾司洛尔及舒芬太尼用于颈丛阻滞的麻醉效果及其安全性。方法选择ASAⅠ-Ⅱ级行甲状腺腺瘤手术病例80例,颈丛阻滞药液为1%利多卡因加0.25%罗哌卡因混合液,总量30ml。随机分为A、B、C、D4组,A组在颈丛阻滞药液中加入艾司洛尔1mg/kg;B组加舒芬太尼0.2μg/kg;C组加艾司洛尔1mg/kg、舒芬太尼0.2μg/kg;D组(对照组)不加药物。观察阻滞后5、15、30min时血压(BP)、心率(HR)和血氧饱和度(SpO2),并行麻醉满意度评分及记录不良反应。结果颈丛阻滞后15-30min,与颈丛阻滞前比较,A、B、D组的血压和心率均明显增高(P<0.05),C组的血压和心率相对平稳,无明显变化(P>0.05),B、C组麻醉满意度明显优于A、D组(P<0.05)。结论低浓度罗哌卡因复合艾司洛尔及舒芬太尼用于颈丛阻滞的麻醉效果满意度高,血流动力学稳定,是一种安全、有效的麻醉方法。 Objective To observe the safety and efficacy of low concentration ropivaeaine combined with esmolol and sulfentanyl in cervial plexum block. Methods The cervial plexum block physic liquor was comprised of 1% lidocaine plus 0.25% ropivacaine with a total volume of 30ml. 80 ASA Ⅰ - Ⅱpatients underwent thyroid surgery were randomly allocated to four groups ( A, B 、 C and D) : group A : esmolol 1 mg/kg was added to cervial plexum block physic liquor; group B: sulfentanyl 0.2μg/kg was added to cervial plexum block physic liquor; group C: both esmolol 1 mg/kg and sulfentanyl 0. 2μg/kg were added to cervial plexum block physic liquor; group D: no additional drugs were added to cervial plexum block physic liquor. Blood pressure, heart rate and SpO2 at time of five minutes, 15 minutes and 30 minutes after cervial plexum block were recorded. Anaesthesia satisfaction degree and adverse effects were also observed. Results 15 min -30 min after cervial plexum block, the BP and HR in group A, B and D were significantly increased ( P 〈 0.05 ) , while that in group C were relatively stable. Anaesthesia satisfaction degree in Group B and C were better than that in Group A and D ( P 〈 0. 05). Conclusion Low concentration repivacaine combined with esmolol and sulfentanyl in cervial plexum block is a safe and effective method with higher anaesthesia satisfaction degree and stable hemodynamics.
