N-乙酰半胱氨酸对机械通气致大鼠急性肺损伤的干预作用

Effects of N-acetylcysteine on the Expression of Transforming Growth Factor-β_1 in Lung of Rats in High Tidal Volume Mechanical Ventilation

摘要 目的通过观察N-乙酰半胱氨酸(NAC)对大潮气量机械通气大鼠肺组织转化生长因子β(1TGF-β1)的影响,探讨NAC对机械通气致大鼠急性肺损伤的干预作用。方法24只雄性Wistar大鼠随机分为对照组、大潮气量组和NAC干预组,测定其肺组织和血浆中丙二醛(MDA)含量,测定支气管肺泡灌洗液(BALF)中蛋白含量,分别采用原位分子杂交技术和免疫组织化学染色法检测肺组织TGF-β1mRNA及其蛋白的表达水平。结果大潮气量组和NAC干预组大鼠肺组织和血浆中MDA含量、BALF蛋白含量和细支气管上皮细胞TGF-β1mRNA及其蛋白表达水平均明显高于对照组(均P<0.01);NAC干预组大鼠肺组织和血浆MDA含量、BALF蛋白含量和细支气管上皮细胞TGF-β1mRNA及其蛋白表达水平明显低于大潮气量组(均P<0.01)。相关性分析结果表明,大潮气量组和NAC干预组大鼠BALF蛋白含量与细气道上皮细胞TGF-β1mRNA表达水平呈正相关(r=0.98,r=0.98,均P<0.01)。结论氧化/抗氧化失衡参与了呼吸机相关性肺损伤(VALI)的发病过程,NAC通过其抗氧化作用对大鼠VALI模型具有一定保护作用。 Objective To explore the antioxidation of N-acetylcysteine ( NAC ) by observing the effect of NAC on the expression of transforming growth factor- β1(TGF- β1) in lung of rats in high tidal volume mechanical ventilation. Methods 24 male Wistar rats were divided into three groups:control group,high tidal volume group and NAC pretreatment group randomly. The levels of MDA in lung and blood and the levels of protein in bronchoalveolar lavage fluid (BALF) were measured. Immunohistochemial staining and molecular hybridization in situ were employed to determine the expression level of TGF-β1 mRNA and protein in pulmonary tissues,following with correlation analysis on the levels of protein in BALF and the expression of TGF- β1 mRNA. Results Compared with control group,the levels of MDA in lung and blood, the protein in BALF and the expression of TGF- β1 mRNA and protein are highly significant(P 〈 0.01 ) in high tidal volume group and NAC pretreatment group. Compared with high tidal volume group,the levels of MDA in lung and blood,the protein in BALF and the expression of TGF- β1 mRNA and protein were decreased significant(P 〈 0.01 ) in NAC pretreatment group. There is obviously positive correlation between the protein in BALF and the expression of TGF- β1 mRNA level in high tidal volume ventilation group and NAC pretreatment group(r = 0.98 and 0.98, respectively, P 〈 0.01 ). Conclusion The imbalance of oxidative/antioxidant may be involved in the pathogenesis of ventilator associated lung injury(VALI). NAC has definite protective effect on VALI by its antioxidation.
