摘要
目的研究静脉滴注卡托普利和呋喃二氢吡啶Ⅰ对急性心肌梗塞(心梗)犬冠脉侧支循环和心梗范围的影响。方法在18只麻醉开胸犬上阻断冠脉左前降支,建立侧支返流模型。卡托普利组(n=6)和呋喃二氢吡啶Ⅰ组(n=6)在缺血5min后分别滴注卡托普利和呋喃二氢吡啶Ⅰ200μg/kg,对照组(n=6)滴注等容积生理盐水。在不同时间点上观察心脏血液动力学指标和侧支循环指标,缺血120min后称重法测定心梗范围。结果两个用药组在用药后心率显著下降。与缺血5min的基础值相比,两个用药组冠脉侧支流量显著增加,侧支系统阻力及外周冠脉阻力显著下降,而对照组无明显变化。两个用药组心梗范围较对照组显著缩小。结论卡托普利和呋喃二氢吡啶Ⅰ均能改善急性心梗犬的冠脉侧支循环,并能限制心梗范围。
Aim To investigate the effects of intravenous infusion of captopril and furyldihydropyriodineⅠ(FDPⅠ) on coronary collateral circulation and infarct size in dogs for 120 minutes. Methods 18 anesthetized openchest dogs were subjected to left anterior descending coronary artery(LAD) occlusion. Retrograde blood flow(Qr) was measured through the ligated and cannulated LAD; collateral blood flow(Qc) was calculated from a load line equation. Captopril and FDPⅠ(200 μg/kg,respectively) and saline were intravenously infused to the dogs after 5minute coronary occlusion. Hemodynamics and collateral circulation indices were measured at different time. Myocardial infarct size was determined with TTC stain technique. Results Qr and Qc were increased markedly, collateral resistance and peripheral coronay resistance were decreased significantly after intravenous infusion of captopril and FDPⅠ, while no significant changes were observed for these indices in control group. Myocardial infarct size was reduced significantly in the two treated groups. Conclusion Captopril and FDPⅠ improved coronary collateral circulation in dogs with acute myocardial infarction and had a beneficial effect on ischemic myocardium.
