正常糖耐量、糖耐量减退及2型糖尿病的冠心病患者胰岛素抵抗和胰岛β细胞功能的差异 被引量:1

摘要 目的:研究正常糖耐量、糖耐量减退及2型糖尿病的冠心病患者胰岛素抵抗和胰岛β细胞功能。方法:选取105例单纯冠心病[糖耐量正常组(NGT组)]患者、39例伴糖耐量减退组(IGT组)和31例伴2型糖尿病组(T2DM组)冠心病患者测量血压、身高、体重、腰围、臀围、血脂、空腹及糖负荷后2h血糖及胰岛素等参数,计算出体重指数、腰臀围比值、胰岛素敏感指数(ISI)、胰岛素抵抗指数(HOMA-IR)和胰岛素分泌指数[胰岛β细胞分泌功能指标(HOMA-IS)及胰岛素分泌指标(FBCI)],各组间进行统计学比较。结果:冠心病患者均存在超重或肥胖、高血压,伴不同程度的高脂血症;其中IGT组和T2DM体重较NGT组重,甘油三酯(TG)较NGT组高,高密度脂蛋白胆固醇(HDL-C)较IGT组低;T2DM组较NGT组更重。IGT组和T2DM组与NGT组比较,IGT组和T2DM组ISI、HOMA-IS和FBCI下降,HOMA-IR增强;T2DM组HOMA-IS、FBCI较IGT组降低。结论:IGT组和T2DM组与NGT组比较,IR更强,胰岛β细胞功能下降;在临床上,T2DM组和IGT组聚集更强的致冠心病危险因子。 Objective:To study insulin resistance and pancreatic E-cell function of coronary heart disease patients with normal glucose tolerance, impaired glucose tolerance or type 2-diabetes. Methods: 175 subjects with coronary heart disease were divided into three groups: 105 subjects with normal glucose tolerance (NGT) group, 39 subjects with impaired glucose tolerance (IGT) group and 31 subjects type 2 diabetes(T2D'M) group. The blood pressure (BP), height, weight, waist circumference, hip circumference and the levels of plasma lipids, glucose, insulin were measured to determine the Body Mass Index(BMI), waist/hip ratio(WHR), insulin resistance level including insulin sen- sitivity index (ISI) and HOMA model insulin resistance index(HOMA-IR) and insulin secretion function including fasting β-cell function index(FBCI) and HOMA model insulin secretion index(HOMA-IS). The statistics were com- pared by these three groups. Results: Obesity, overweight and hypertension generally were seen on the patients of coronary heart disease, and accompany in varying degrees of hyperlipidemia. IGT, T2DM groups were heavier in weight, triglyceride (TG) level were higher, high density lipoprotein cholesterol(HDL-C)level were lower and ISI, HOMA-IS,FBCI were smaller than NGT group. T2DM group was heavier in weight, lower in HOMA-IS and FBCI than IGT group. Conclusion: Higher insulin resistance, worse pancreatic β-cell function in IGT and T2DM groups have powerful causative agent of coronary heart disease than those in NGT group.
