血必净注射液对呼吸衰竭患者氧代谢的影响 被引量:9

摘要 目的:探讨血必净注射液对肺部感染、呼吸衰竭患者氧代谢及预后的影响。方法:将71例呼吸衰竭重症患者随机分为血必净治疗组与对照组:治疗组加用血必净100ml静脉点滴,每天2次,共7d,其余治疗两组相同。于机械通气前和机械通气第1、2、3天取动脉与中心静脉血检查血气分析,入室当天与机械通气后第3天取静脉血查CRP。结果:治疗后第1天始两组缺氧即明显改善(P<0.01)。组织氧合方面:治疗后第2、3天中心静脉氧饱和度(ScvO2)两组较前均有所改善(P<0.01),组间比较亦有极显著性差异(P<0.01);动脉血乳酸(Lac)治疗组治疗后第2、3天明显降低(P<0.01),组间比较亦有极显著性差异(P<0.01)。治疗组CRP较前明显降低(P<0.05)。治疗组治疗5d脱机成功及14d好转出科(转科或出院)例数明显高于对照组,两组比较有显著性差异(P<0.05)。结论:血必净可较好地改善肺部感染、呼吸衰竭患者的氧代谢。 Objective: To observe the effects of Xuebijing injection on oxygen metabolism in patients with lung infection and respiratory failure, and explore the possible mechanism. Methods: 71 critical patients with respiratory failure were randomly divided into two groups: observation (Xuebijing) group, 36 patients treated with routine treatment and intravenous drip of Xuebijing, and control group, 35 patients treated with routine treatment. Patients' clinical status and outcome were observed, meanwhile the blood lactate concentration and the blood gas of arterial and central venous blood were monitored on the day 0, 1, 2, 3 respectively. CRP level of venous blood was monitored on the day 0, 3 respectively, too. Results: Hypoxemia had been improved since the first day in both group (P〈0.01). The central venous oxygen saturation (ScvO2) was apparently increased on the day 2,3 in treatment group compared to control group (P〈0.01). The arterial lactate concentration (Lac) was apparently decreased on the day 2,3 in treatment group compared to control group(P〈0.01). The level of CRP was markedly decreased on the third day after treatment in treatment group (P〈0. 05). The cases of ventilatory weaning at the day 5 and transferring into other department after improvement at the day 14 in treatment group were more than those in control group (P〈0.05). Conclusion: Oxygen metabolism is markedly improved in patients with lung infection and respiratory failure by Xuebijing treatment. The curative effect of Xuebijing is good.
