螺旋CT三维重建诊断复杂性骨盆骨折 被引量:13

The Value of Multi - mode 3D Reconstruction by Spiral CT in Diagnosis of Complex Pelvic Fracture

摘要 目的:探讨螺旋CT(SCT)多模式三维重建(MM3DR)诊断复杂性骨盆骨折的应用价值。方法:128例均摄骨盆C线片;同时采用3—5mm层厚连续SCT扫描,1.5~2.5mm间距后重建,源像传于AW4.0工作站行SSD、MPR及MIP三维重建;采用SSPS13.0软件分析。结果:SCTMM3DR与X线片对复杂性骨盆骨折诊断率差异有统计学意义(P〈0.01);SSD与MIP能对复杂骨盆骨折获取清晰全景立体图像,MPR能多角度观察骨盆各骨骨折的细节情况。结论:MM3DR对复杂骨盆骨折诊断、分型与手术选择等有重要参考价值。 Objective :To explore clinical application of multi -mode 3D reconstruction (MM3DR)by using spiral CT(SCT) in diagnosis of complex pelvic fractures. Methods :One hundred and twenty eight cases with pelvic fractures accepted X -ray test and continuous SCT with thickness of 3 -5mm in each slice, 1.5 - 2.5mm between consecutive two slices. The original images were uploaded to AW 4.0 workstation and then were reconstructed to be SSD ( surface shadow display) , MIP ( Maximum Intensity Project) and MPR ( Multiplanar reformation). These data were analyzed by the software of SSPS13.0. Results:There was significant difference in diagnosis as pelvic fracture by 3DSCT and X - ray test ( χ^2 = 35. 758 ,P 〈 0.01 ). SSD and MIP could display the full view of stereopicture of complex pelvic fracture. The details of pelvic fracture can be observed on MPR. Conclusions :MM3DR has important values for diagnosis, classification and surgical setected of complex pelvic fracture.
