摘要 目的:研究胎盘组织超微结构的病理变化、呼吸链酶复合体活性变化与子痫前期的关系。方法:选择轻度子痫前期孕妇30例(轻度子痫前期组)、重度子痫前期孕妇15例(重度子痫前期组)和正常孕妇20例(正常妊娠组)行胎盘透射电镜观察,比较两组超微结构,同时采用紫外分光光度计法测定胎盘组织线粒体中两种呼吸链酶复合体(线粒体酶复合体Ⅰ、Ⅳ)的活性进行分析。结果:轻度子痫前期组与正常妊娠组胎盘超微结构比较,差异无统计学意义(P>0.05),重度子痫前期组与正常妊娠组和轻度子痫前期组胎盘超微结构比较,差异有统计学意义(P<0.05);重度子痫前期组胎盘组织中线粒体酶复合体Ⅳ的活性比正常妊娠组、轻度子痫前期组低,差异有高度统计学意义(P<0.01),轻度子痫前期组线粒体酶复合体Ⅳ的活性与正常妊娠组相比,差异无统计学意义(P>0.05)。线粒体酶复合体Ⅰ在3组间差异无统计学意义(P>0.05)。结论:子痫前期患者胎盘超微结构的变化、线粒体酶复合体Ⅳ活性降低,提示子痫前期发病机制中有能量代谢传递障碍,线粒体能量产生异常参与子痫前期的发病过程。 Objective: To study the relationship between the preeclarnpsia and the ultra-structure pathological changes of placenta, the activities of respiratory chain enzymes. Methods:The ultra-structure of placental villi under transmission electron microscope and the activities of respiratory chain enzymes ( mitochondrial enzymes complex Ⅰ and Ⅳ) in mitochondria by ultraviolet spectrophotometer were detected in mild 30 preeclampsia patients (mild preeclampsia group), severe 15 preeclampsia patients (severe preeclampsia group) and 20 healthy pregnant women (control group).Results: There was no statistic difference in the ultra-structure of placental villi between mild preeclampsia group and the control group( P 〉 0.05). There was statistic difference in the ultra-structure of placenta between sever preeclampsia group and mild preeclampsia group, the control group ( P 〈 0.05). The activity of mitochondrial enzymes complex Ⅳ was significantly lower in the severe preeclampsia group than in the mild preeclampsia and the control group( P〈 0.01 ). There was no statistic difference in the activity of mitochondrial enzymes complex Ⅳ between the mild preeclarnpsia group and control group( P〉0.05). Mitochondria enzymes complex Ⅰ was no significant changed in the three groups( P 〉 0. 05). Conclusions:The changes of ultra-structure of placentas especially decreasing activities of enzymes complex Ⅳ in preeclampsia suggest that the dysfunction of energy metabolism can induce preeclampsia. The abnormally of the energy produced in mitochondrion may be involved in the onset of preeclampaia.
