摘要
目的:探讨改善下睑袋整复手术效果的方法。方法:在传统外切入路的下睑袋成形术中,分离下睑皱纹区皮瓣,在紧缩眶隔、眶肌筋膜韧带眶外缘骨膜提紧固定的基础上,于外眦外侧将下睑皮瓣外上方提紧略有张力缝合固定于眶外缘骨膜,双眼向头侧注视术者,下睑最大张力状态下去除多余下睑皮肤。结果:本组37例患者,其中35例患者获随访3～17月,1例隐裂处切口痕迹较明显外,其余34例下睑区皮肤平坦紧致,无睑外翻、下睑退缩、外眦下垂等并发症,对下睑皱纹的去除及中面部提升效果明显,医患满意。结论:皮瓣、眶肌筋膜瓣分层提紧的睑袋成形术可同步恢复下睑各松弛结构的张力,全面加强下睑支持结构,皮肤去除量准确、可靠,有效防治睑外翻及矫正不足,患者满意度提高。
Objective To explore the way to improving the effect of lower lid blepharoplasty. Methods During the Lower eyelid blepharoplasty by typical outside approach, wrinkle region skin flaps was separated, based on fascia palpebralis tighten as well as outer margin periost of fascia ligament of orbital muscle lifted and fixed, outside and upper lower eyelid skin flaps was lifted tightly and sewed to orbital outer margin periost with little tension on the lateral temporal angle, and patients＇eyes stare at operator on the side of head, who＇s lower eyelid superfluous skins were cut in the state of lower eyelid maximal tension. Results Among this group of 37 cases patients, 35 patients were followed up for 3-17months.Except one had conspicuous cut trace in subfissure, the rest＇s lower eyelid skins were plant and conpact without ectropion, lower eyelid retraction, temporal angle ptosis,ect. Obviously effect of lower eyelid wrinkle＇s removal and middle face lifted satified doctors and patients. Conclusions Lower eyelid blepharoplasty by layered and lifted tightly skin flaps and fascia flaps of orbital muscle enabled each loose constitution＇s tension to recovery at equal pace, and enhance lower eyelid supporting structure completely,and skin was cut exactly and practically, by which can prevent and cure ectropion and undercorrection effectively. Patient contented well.
