摘要
目的探讨多层螺旋CT血管成像术在下肢动脉闭塞性疾病中的诊断价值。方法对63例临床可疑的外周动脉闭塞性疾病患者行下肢动脉多层螺旋CT血管造影(CTA),扫描范围从肾动脉水平达足底。薄层重建横断面图像传入HP xw8000工作站,进行血管三维重建。结合常规血管造影(DSA)评价CTA诊断的准确性。结果在1 323个动脉节段中,1 305个节段在CTA与DSA均可以显示,DSA显示狭窄和闭塞的节段共390个,其中轻度狭窄48个,中度狭窄36个,重度狭窄66个,闭塞240个;CTA显示狭窄和闭塞的节段共393个,其中轻度狭窄51个,中度狭窄30个,重度狭窄72个,闭塞240个。最大密度投影(M IP)显示CTA对中度及中度以上狭窄诊断的敏感性、特异性及准确性分别为99.1%、99.7%和99.5%。结论多层螺旋CT在评估下肢动脉闭塞性疾病方面与常规血管造影结果无明显差别,是下肢动脉闭塞性疾病理想的影像学检查手段。
Objective To evaluate the diagnostic value of multi-slice spiral CT in assessing the lower ex- tremity arterial occlusive diseases. Methods Sixty-three patients with suspected peripheral arterial occlusive diseases underwent multi-slice spiral CT angiography (CTA) of lower extremity arteries, the helical scans cover renal artery to the artery of the lower extremity. HP xw8000 workstation was used for 3D reconstruction. Digital subtraction angiography (DSA) was used as the standard reference, the diagnostic accuracy of CTA was determined. Results 390 stenoocclusive Of the 1 323 segments, 1 305 were assessable on both DSA and CTA. On DSA images, segments (48 mild stenosis, 36 moderate stenosis, 66 severe stenosis, and 240 occlusion) were detected. On CTA images,393 stenoocclusive segments (51 mild stenosis,30 moderate stenosis,72severe stenosis, and 240 occlusion) were detected. With regard sensitivity, specificity and accuracy of maximum intensity Conclusion multi-slice spiral CT has sensitive, specific to segments that had more than moderate stenosis, the projection(MIP) were 99.1% ,99.7% and 99.5%. and accuracy in detecting lower extremity arterial occlusive diseases and may partially substitute DSA.
