血清铁蛋白与骨髓可染色铁的相互关系

摘要 用放射免疫法测定了78名健康成人的血清铁蛋白(SF)浓度,其中44个男性的SF为88.7±41.0μg/L,34个女性的SF为49.0±24.7μg/L.78个各种不同原因所致贫血病人的SF浓度为1.5~399μg/L,骨髓穿刺物的涂片中可染色铁的含量可划分为5个等级(0~4+).这两项检测所得结果呈高度正相关,等级相关系数rs=0.79,P<0.005,但各组间SF浓度有交叉重叠,个别病例两种检测方法所得结果相差悬殊.铁粒幼细胞百分数与SF亦呈正相关,rs=0.67,P<0.001.SF浓度反映体内铁贮存量比骨髓可染色铁含量更加准确,更有价值.对SF浓度测定的优点进行了讨论. Serum ferritin concentration was determined by means of radioimmunc assay technique in 78 healthy adults with results of 88.7±41.0μg/L in 44 men and 49.0±24.7μg/L in 34 women. In 78 patients with varying degrees of anemia of different etiologies the serum ferritin levels varied from 1.5 to 399μg/L, while the stainable iron in smears of bone marrow aspirates was scaled into 5 grades from 0 to 4^+. The correlation coefficient(γs) between the serum ferritin and marrow iron was 0.78(P<0.0005), but there was an overlap between the groups and in a few exceptional cases there was a d(?)sparity between the two values. There was also a close correlation between serum ferritin concentration and percentage of sideroblasts with γs=0.67, P<0.001. Serum ferritin determ(?)mation is more accurate and valuable in the estimation of iron deposit in the body. Its advantages are d(?)scussed.
