摘要
应用多普勒超声研究64例二尖瓣位机械瓣瓣口血流动力学指标的动态演变、影响因素及其与房室功能的关系。发现相同型号的机械瓣在不同心率、瓣膜返流、心排量状态下测量的有效瓣口面积、峰值和平均跨瓣压差、峰值和平均瓣口流率有显著差异,而峰值和平均瓣口阻力(PR和MR)无显著差异;换瓣前后MR的实测值和变化率与左房张力(LAT)的实测值及变化率的相关性均较前5项指标为佳;相同型号不同瓣叶类型(单叶与双叶)的机械瓣PR和LAT有显著差异,相同瓣叶类型不同型号(25号与27号)的机械瓣之间各指标无显著差异;但时间-MR曲线偏离。换瓣≥5年组较<5年组的MR和LAT等有显著差异。结论:二尖瓣位机械瓣瓣口功能状态存在类型差异和时间衰退,MR是机械瓣功能衰退程度动态评价的稳定而可靠的指标。
Dynamical changes, influential factors and their associations with left atrial and ventricular function of 64 cases of mechanical valve in the mitral valve position were studied in this paper. A significant difference of whiral valve area (MVA), peak and mean pressure gradient and peak and mean valvular flow rate Qmax and Qmean) but no significant difference of peak and mean valve resistance (PR and MR) under the conditions of different heart rates, valve regurgitation and cardiac output in same number of mechanical valves was found. Practical values and change rates of MR before and after mitral valve replacement (MVR) had a best relation with those of left atrial tension (LAT) when comparison with 5 previous indexes PR and LAT were significant difference between different valvular types (single leaf valves and double - leaf valves) of same valve numbers.All indexes had no singificant difference but their Time - MR curves diverged between different valve numbers (No. 25 valves and No. 27 valves) of same valve types. MR and LAT had a significant difference between the group of MVR < 5 years and the group of MVR ≥ 5 years. Conclution:There is a type difference and time decline of valvular function of mechanical valves in the theiral valve position MR is a steady and reliable index of dynamical evaluation of decline severity of valvular function of prothetic valves.
