摘要 目的探讨应用双筋膜蒂耳后皮瓣一期修复大块耳廓组织缺损的手术方法及其疗效。方法利用耳后双筋膜蒂皮瓣修复大块耳廓组织缺损患者11例,一期进行耳轮、耳舟区精细结构塑形,其中最大修复面积达到3.2cm×1.6cm。结果术后所有皮瓣存活,随访3~9个月,耳廓外形良好。结论耳后区双筋膜蒂皮瓣修复大块耳廓组织缺损的方法,手术操作简单、易行,可提供较大的组织瓣修复。修复后耳廓组织质地、颜色与正常相近,耳廓外形恢复良好,供瓣区位于耳后,术后瘢痕隐蔽,有较好的临床应用价值。 Objective To study the advantages of using postauricular bi-fascial pedicle flap for one stage reconstruction of large auricular defect. Methods 11 cases of large auricular defects were repaired by postauricular bi-fascial pedicle flaps. The fine structures of helix and scaphoid area were shaped in one stage and the maximum size of defect was 3.2cm×1.6cm, Results All flaps were survived postoperatively. The 3 to 9 months follow-up in 11 patients were resulted in satisfactory appearances. Conclusion The technique of postauricular bi-fascial pedicle flap, which could provide sufficient donor tissues, is a simple and ideal technique for one stage reconstruction of large auricular defect. The reconstructed auricle well matches normal auricular tissue in contour, texture and color, while the inconspicuous scar of donor site is acceptable.
