摘要
以糖尿病管理信息系统为例,介绍了上海市闸北区依托区域卫生信息平台开展慢性病管理的现状及特点。通过卫生信息共享与协同服务,能更加及时的发现患者、优化疾病管理模式、提升管理质量和患者依从性、加强居民自我健康管理、提高工作效率。
This paper introduced current situation and features of chronic disease management based on health informatization in Zhabei district, Shanghai. Take the diabetes management information system as an example. By sharing and synergy of the health information platform, patients can be found in time. Besides, disease management model was optimized. The management quality and compliance of patients were advanced. In addition, the self-management of patients was enhanced and statistical efficiency was improved. The results show that chronic disease management system lays a solid foundation for the achievement of the aim of new healthcare reform to provide everyone with access to primary medical and health services.
