腹内压对大鼠生理指标的影响及腹压监测法相关性研究 被引量:4

摘要 目的了解腹内压(IAP)对大鼠呼吸、循环、肾功能的影响,以及间接测压法是否具有与直接测压法同样的意义。方法将60只SD大鼠随机分成健康对照组、IAP1-5组(IAP值分别为5、10、15、20、25mmHg,1mmHg=0.133kPa),每组10只大鼠。记录各组大鼠呼吸、循环、肾功能的生理指标以及IAP检测值,并将经下腔静脉及膀胱测得的间接测压值与直接测压值进行相关性分析。结果IAP1组大鼠与健康对照组各指标接近;IAP2组PaO2显著降低(P〈0.05),血清肌酐、尿素氮含量显著升高(P〈0.05);IAP3组、IAP4组除呼吸频率、平均动脉压外,其余指标与健康对照组比较,差异均有统计学意义(P〈0.05或P〈0.01);IAP5组PaO2、平均动脉压显著降低(P〈0.01),其余指标升高(P〈0.05或P〈0.01)。2种间接测压值与直接测压值均呈显著正相关(r值分别为0.937、0.955,P〈0.01)。结论IAP对大鼠呼吸、循环、肾功能均有影响,但程度不同;经下腔静脉、膀胱间接测压法可替代损伤较大的直接测压法。 Objective To study the effects of intra-abdominal pressure (IAP) on respiratory system, circulatory system and renal function in rats. To investigate the difference between the direct measure and indirect measure methods (via inferior vena cava and bladder) for lAP. Methods Sixty Sprague- Drawly (SD) rats were randomly divided into 5 groups with different IAP (IAP value of 1-5 groups was 5, 10, 15, 20, 25 mm Hg,respectively) and healthy control group, 10 rats in each group. The parameters of respiratory system, circulatory system, renal function, and IAP value were recorded. The correlation between direct and indirect measurement methods was also analyzed. Results There were no significant differences in above parameters between IAP1 and healthy control groups. Compared with those in healthy control group, PaO2 significantly decreased ( P 〈 0.05 ) , SCr and BUN increased significantly in IAP2 group ( P 〈0.05). Other indexes inIAP3, IAP4 groups increased ( P 〈0.05 or P 〈0.01) except for respiratory frequency(RF) and MAP ( P 〉0.05). PaO2 and MAP decreased ( P 〈0.01), and other indexes increased( P 〈 0.05 or P 〈 0.01 ) in IAP5 group. The values obtained from the indirect measure method were positively correlated with that from the direct measure method ( r = 0. 937, 0. 955, P 〈0. 01, respectively). Conclusions lAP can affect respiratory system, circulatory system and renal function in different degrees in rats. The indirect measure method can replace the direct measure method for lAP measure with little injuries.
