经T管窦道网篮取石治疗肝移植术后早期胆管内胆栓3例

摘要 目的探讨经T管窦道网篮取石在肝移植术后早期胆管内胆栓治疗中的应用价值。方法对3例原位肝移植患者,胆管重建采取胆总管端端吻合,留置T管。术后3个月内,T管造影提示胆管内胆栓形成。拔除T管,经T管窦道用网篮完整取出胆树状铸型胆栓,术后留置引流管。结果3例患者术后肝功能正常。1例患者拔除引流管;2例患者术后存在胆管感染,经抗感染治疗,留置外引流管支撑。结论经T管窦道网篮取栓可早期解除胆管梗阻,该方法简单、经济,可减少患者痛苦,可作为治疗肝移植术后早期胆管内胆栓的一种手段。
