摘要
目的:了解我院临床分离的凝固酶阴性葡萄球菌(CNS)对莫匹罗星的耐药状况,并检测耐药菌株对常用抗菌药物的耐药性。方法:对我院2005~2007年临床分离的表皮葡萄球菌与溶血葡萄球菌用纸片扩散法筛选莫匹罗星耐药菌株,以最小抑菌浓度(MIC)确定其耐药水平,PCR检测mupA耐药基因,同时纸片扩散法检测莫匹罗星耐药菌株对头孢西丁、庆大霉素、左氧氟沙星、复方新诺明、利福平、红霉素、克林霉素、四环素及万古霉素的耐药性,对头孢西丁耐药菌株进行mecA基因扩增。结果:431株CNS共筛选出64株莫匹罗星耐药菌株,其中表皮葡萄球菌31株[27株为高水平莫匹罗星耐药(MuH),4株为低水平莫匹罗星耐药(MuL)]、溶血葡萄球菌33株(均为MuH)。31株莫匹罗星耐药表皮葡萄球菌中有21株耐甲氧西林,33株溶血葡萄球菌中有23株耐甲氧西林。莫匹罗星耐药CNS对临床常用抗菌药物的耐药性较为严重,溶血葡萄球菌尤为突出,且MuH菌株的耐药率普遍比MuL菌株高。但利福平对莫匹罗星耐药CNS具有较好的抗菌活性,未发现万古霉素耐药菌株。结论:莫匹罗星耐药CNS在我院临床分离株中已有较高的分离率,以高水平耐药为主,并对临床常用抗菌药物耐药率高并呈多重耐药性。
Objective:To investigate the resistance to mupirocin in coagulase-negative Staphylococcus(CNS) isolated from our hospital,and test the resistances to commonly used antibiotics in clinic among mupirocin-resistant strains.Methods:The strains of clinically isolated Staphylococcus epidermidis and Staphylococcus haemolyticus collected from January 2005 to may 2007 were screened for mupirocin susceptibility using disc diffusion method.The minimum inhibition concentration(MIC) and the amplification of mupA gene were performed to determine the resistance level to mupirocin.We also tested the resistences to cefoxitin,gentamycin,levofloxacin,trimethoprim/Sulfamethoxazole,rifampicin,erythromycin,clindamycin,tetracycline and vancomycin by disc diffusion method among the mupirocin-resistant strains.Amplification of mecA gene was performed among cefoxitin-resistant strains.Results:Among 431CNS strains,64 mupirocin-resistant strains were identified,which comprised 31 isolates of S.epidermidis and 33 isolates of S.haemolyticus.Among the isolates of S.epidermidis 27 isolates demonstrated high-level resistance to mupirocin,the rest demonstrated low-level,while 33 isolates of S.haemolyticus were all high-level resistance to mupirocin.31 isolates of S.epidermidis included 21 Methicillin resistant isolates,while 33 isolates of S.haemolyticus included 23 isolates.Resistance rates to most antibiotics of mupirocin-resistant CNS were grave,especially S.haemolyticus.The drug resistance rates of MuH strains were higher than MuL strains.Rifampicin remained relatively sensitive.No vancomycin-resistant strains were found.Conclusion:The mupirocin-resistant CNS strains have emerged in our hospital and the isolation rate was high.Antibiotics resistance of the isolates was severe and demonstrated multidrug resistance.
