摘要
目的探讨nm23、CD44v6在甲状腺肿瘤组织的表达情况及其与肿瘤发生、发展、浸润和转移的关系。方法采用免疫组织化学SP两步法检测70例甲状腺癌、10例癌旁甲状腺组织和10例甲状腺腺瘤的石蜡标本nm23、CD44v6表达水平。结果甲状腺癌中nm23蛋白阳性表达率72.9%,各病理类型间阳性率差异无显著性,nm23蛋白阳性率与性别、年龄无相关性,nm23蛋白细胞浆阳性与淋巴结转移无统计学相关性,nm23蛋白细胞膜阳性与甲状腺癌的复发、远处转移相关。甲状腺癌中CD44v6蛋白阳性率77.1%,显著高于甲状腺腺瘤者40.0%,癌旁甲状腺组织未见表达,甲状腺癌中不同组织来源的滤泡细胞癌和髓样癌的CD44v6表达差异具有显著性(P<0.05),CD44v6表达水平在淋巴结转移组和非转移组间差异有显著性(P<0.05),CD44v6表达与性别、年龄无相关性。在转移、复发组与非转移、复发组间比较差异有显著性。结论nm23、CD44v6的表达与甲状腺肿瘤的分化和浸润、转移显著相关,检测二者有助于甲状腺癌的诊断、预后判断、并可能为甲状腺癌的手术治疗决策提供新的分子生物学指标。
[Objective] To investigate the expression of the cell adhesion molecule （CD44v6）, and rim23 in thyroid tumor tissue and its relationship with the pathogengsis, development, invasiveness and metastasis of the tumor. [Methods] Immunohistoehemistry was apphed to examine the expression of CD44v6 and nm23 in 10 specimens of tissue adjacent to carcinoma, 10 of thyroid adenoma and 70 of thyroid carcinoma all the specimens were paraffin- embedded. [Result] The positive rate of nm23 was 72.9%. The difference of the various pathologic types of the tumor was not significant statistically. It had no relation with age and sex. The nm23 protein cytoplasmic staining observed no statistical significance. But membrane staining correlated with metastasis and recurrences. The expression of CD44v6 was observed in 4 specimens of adenomas （10）, and 54 of carcinoma （70）, there were significant differenee between adenomas and carcinoma. But was not detected in the tissue adjacent to carcinoma. The expression of CD44v6 was significantly different between follicular epithelial carcinoma and medullary carcinoma （80.3% vs 25.0%）. The frequency of overexpression of CD44v6 was associated with metastasis, recurrences and lymph node metastasis （P 〈0.05）.But it had no relation with age and sex （P 〉0.05）. [Conclusion] It is suggested that CD44v6 and nm23 will he useful for the diagnosis of thyroid carcinoma and serves as a biological marker for local invasion and lymph metastasis of thyroid carcinoma.
