吡硫翁锌气雾剂治疗寻常型银屑病的系统评价 被引量:2

Systematic review of therapeutic effect of pyrithione zinc aerosol on treating psoriasis vulgaris

摘要 目的:系统评价吡硫翁锌喷雾剂治疗寻常型银屑病的疗效和安全性。方法:计算机检索Medline(1966~2008.6)、Ovid(1996~2008.6)、The Cochraneli brary(2008年第6期)和维普全文数据库(1990~2008.6),纳入吡硫翁锌治疗寻常型银屑病的随机对照试验。方法学质量分析依据Cochran Reviewers’Handbook,进行Jadad评分,并应用RevMan4.2软件进行Meta分析。结果:共纳入5篇随机对照试验(RCT),其中3篇为吡硫翁锌气雾剂与羟基喜树碱软膏治疗寻常型银屑病的对照试验。结果显示吡硫翁锌气雾剂治疗寻常型银屑病较羟基喜树碱软膏更有效,且不良反应发生率更低。1篇为吡硫翁锌与派瑞松的对照试验,其4周治疗的有效率无统计学差异,但8周、12周之后,吡硫翁锌治疗的有效率较高,且其不良反应的发生率低于派瑞松。1篇为吡硫翁锌和丙酸氯倍米松合用的对照试验,结果显示其联合用药并不能提高治疗有效率。结论:吡硫翁锌治疗寻常型银屑病安全、有效,长期治疗效果优于羟基喜树碱软膏和派瑞松。丙酸氯倍米松与吡硫翁锌联合并不能提高疗效。但尚需更多的RCT证实其结论的可靠性。 Objective: To evaluate the therapeutic effect and security of pyrithione zinc on psoriasis (1966-2008.6^th), OVID (1996-2008.6^th), The Cochrane library (2008.6^th) and Vip ( 1990-2008.6^th) were collected. Methodological quality and data extraction were assessed by Cochran Reviewers'Handbook and the criteria of Jadad. RevMan 4.2 was used for Meta-analysis. Results: There were 5 papers of randomized clinical trial in all. Three of them were randomized controlled trial on pyrithione zinc aerosol and hydroxycamptothecine (OPT). The systematic review showed that there were statistic difference between topic treatment with pyrithione zinc aerosol and OPT ointment group. One of them was randomized controlled trial on pyrithione zinc aerosol and Pevisone. It showed that these two medication had the same effectiveness for the first 4 weeks. But for 8 and 12 weeks, pyrithione zinc aerosol group had better therapeutic efficacy and fewer side effect. Another paper was randomized controlled trial on drug combination of Clobetasol with pyrithione zinc aerosol. It showed that pyrithione zinc aerosol could not improve the therapeutic efficacy of Clobetasol. Conclusion: It is effective on treating psoriasis vulgaris using pyrithione zinc aerosol. And there are fewer side effects using pyrithione zinc than OPT ointment and Pevisone. Drug combination of Clobetasol with pyrithione zinc aerosol could not improve the therapeutic efficacy. But the high-quality study is still required.
