摘要
目的探讨带旋髂深血管髂蒂骨瓣移植治疗成人股骨头缺血坏死的方法及疗效。方法2003年1月至2007年7月对12例股骨头缺血坏死采用带旋髂深血管蒂的髂骨瓣移植治疗。结果术后12例获得随访1—5年，平均3．2年。根据范启申的评定方法：优6例，良4例，中2例，差0例，优良率83．3％。结论带旋髂深血管蒂的髂骨瓣手术治疗成人股骨头缺血坏死具有重建股骨头血运、髓芯减压、清除坏死骨和支撑股骨头软骨的特点。带旋髂深血管蒂的髂骨瓣手术治疗是治疗成人股骨头缺血坏死的有效方法。
Objective To investigate the treatment method of ischemic necrosis of the femoral head by the transplantation of pedicled bone flap with deep iliac eireum flex vessel in adult and to assess its curative effect. Methods From January 2003 to July 2007, 12 cases ischemic necrosis of the femoral head were treated by tronsplantation of pedieled bone flap with deep iliac cireum flex vessel. Results 12 cases were followed up one year to five years after operation, on the average 3.2 years. According to the assess of Fan Qishen, the result was excellent in 6 cases, good in 4 cases, fair in 2 cases and poor in 0 eases. The postoperative excellent and good rate was 83.3%. Conclusions Operative treatment of ischemie necrasis of the femoral head in adult by transplantation of bone flap pedicled with deep iliae circum flex vessel has the characteristics of restoration of blood supply of the femoral head, decompression of marrow cavity, elimination of necrotic bone and support of the femoral head. It is an effective and advanced method for treatment of isehemic necrosis of the femoral head in adult.
