摘要
目的:近年研究表明,炎症反应在动脉粥样硬化及其并发症的发生、发展中起着重要作用。观察冠状动脉支架置入后支架内血栓形成患者牙周状况和龈沟液中前列腺素E2、肿瘤坏死因子α、白细胞介素1,13水平变化。方法:选择2002-02/2008-05在辽宁医学院第一临床学院心内科行冠状动脉内支架置入后行冠状动脉造影复查的患者315例,其中出现支架内血栓形成的患者47例。比较冠状动脉支架置入后支架血栓形成患者与支架置入后正常患者的牙周状况,应用酶联免疫法测定龈沟液及血清中前列腺素E2、肿瘤坏死因子α、白细胞介素1,13水平。结果:支架内血栓形成患者牙周病的发病率显著高于支架正常的患者(41.7%,7.9%,P<0.01)。支架内血栓形成患者血清中前列腺素E2、肿瘤坏死因子α水平显著高于支架正常患者(P<0.01),白细胞介素1,13水平与支架正常患者差异无显著性意义(P>0.05)。支架内血栓形成患者龈沟液中前列腺素E2、肿瘤坏死因子α及白细胞介素1水平显著高于支架正常患者(P<0.01),白细胞介素13水平低于支架正常患者(P<0.01)。结论:牙周炎患者发生支架内血栓形成的概率明显高于未患牙周炎的患者,因此应加强冠状动脉内支架置入患者口腔疾病的防治。
AIM: Recent studies have shown that inflammatory reaction plays an important role in atherosclerosis and the incidence and progression of its complications. This study detected the change of the level of prostaglandin E2 (PGE2), tumor necrosis factor alpha (TNF-α), interleukin-1 (IL-1) and IL-13 in gingival crevicular fluid and the periodontal condition of patients undergoing coronary stenting.
METHODS: 315 patients undergoing coronary arteriography following coronary stenting in Department of Cardiology, First Clinical College of Liaoning Medical University between February 2002 and May 2008 were selected, and 47 of them developed in-stent thrombus. The periodontal condition was compared between the patients with thrombus in stent and normal patients undergoing coronary stenting. The ELASE was used to detect the level of PGE2, TNF- α, IL-1 and IL-13 in the serum and gingival crevicular fluid.
RESULTS: The incidence of periodontal disease in patients with thrombus in stent was significantly greater (41.7%, 7.9%, P 〈 0.01) and the level of PGE2 and TNF- α in serum was higher than patients without thrombus (P 〈 0.01). However, no significant difference was found in IL-1 and IL-13 between two groups (P〉 0,05). The levels of PGE2, TNF- α and IL-1 in gingival crevicular fluid were significantly greater in patients with thrombus in stent than patients with no thrombus (P 〈 0.01), while the level of IL-13 was lower than patients with no thrombus (P 〈 0.01).
CONCLUSION: The incidence of thrombus in stent is high in patients with periodontal disease, so oral disease prevention is necessary for patients undergoing coronary stenting.
