摘要
对332例糖尿病患者肢体神经传导速度(NCV)检测结果及临床情况进行回顾性分析。结果显示:Ⅰ型糖尿病48例中,NCV异常率为77.1%,略高于Ⅱ型糖尿病284例中的66.2%(P>0.05),总异常率为67.8%。Ⅱ型糖尿病者中病程>5年组的NCV阳性率(74.5%)高于≤5年组(58.0%,P〈0.05)。332例共检测神经2546条,其中运动神经传导速度(MCV)检测异常率(37.0%)和感觉神经传导速度(NCV)检测异常率(35.3%)相似(P>0.05)。各例检测NCV受损程度及神经分布差异很大。两侧肢体对应检测的740对神经NCV,结果一致的占88.6%,不一致的占11.4%。仅检测单侧肢体神经NCV的1066条异常率33.8%,较双侧异常率43.6%为低(P〈O.05)、有症状肢体SCV检测异常率较无症状肢体SCV异常率高(P〈O.05)。结果提示,加强有症状肢体及双侧同时检测有助于NCV异常的检出,减少糖尿病神经病变的漏诊率。
The nerve conduction velocity (NCV) of extremities and its related influential factors in 332 diabetic patients were evaluated retrospectively. The results showed that the rate of NCV impairment (RNI) was totally 67. 8%, with 77. 1 % in type l diabetes mellitus(48 cases) and 66. 2% in type 2 diabetes mellitus(284 cases). There was no significant difference between them (P > 0. 05 ). NCV was determined in a total of 2546 nerves. The RNI of motor NCV(MCV)was 37 %, similar to that of sensory NCV (SCV) 35. 3%, P > 0. 05. There was 11. 4% showing only unilateral nerve impairment among the 740 pairs of nerves determined bilaterally. 43. 6% of RNI in the 740 pairs of nerves was higher than the 33. 8% in the 1066 nerves determined unilaterally (P <0. 05). The RNI of SCV in patients with clinical signs was higher than that without any signs. The present data suggest that it's important to determine NCV bilaterally and in symptomatic limbs, so as not to miss the diagnosis of diabetic neuropathy.
