大鼠实验性骨关节炎模型的建立及病理特征 被引量:27

摘要 目的通过前交叉韧带切断建立大鼠骨关节炎模型并观察其病理特征,为骨关节炎的研究提供经济有效的动物模型。方法体重200~250 g的3月龄Wistar大鼠雌雄各20只,随机选取30只作为实验对象,每只随机选取一侧膝关节作手术侧。苯巴比妥钠麻醉后常规备皮消毒膝关节,取髌旁内侧切口显露膝关节髌骨后,将髌骨向外侧脱位,尽量屈曲膝关节显露前交叉韧带,在直视下切断前交叉韧带,将髌骨复位,闭合关节腔,对侧不手术作为空白对照组,大鼠不作任何固定措施,分笼饲养。另10只作为假手术对照。术后3 d每天每只大鼠肌肉注射青霉素20万单位预防感染。分别于术后2、4及8周,随机处死10只大鼠,对软骨退变情况行大体观察及光镜观察。结果实验组2周时,关节软骨失去正常光泽,软骨面不规则;4周时,10例中有4例出现关节积液,并可见滑膜增生,软骨面粗糙糜烂,色泽灰暗;8周时,10例中有8例出现关节积液和滑膜增生,软骨面不规则,出现溃疡甚至软骨缺损,软骨下骨质裸露。光镜下改变:2周后,可见软骨表面原纤维形成,表层软骨细胞肿胀变圆,扁平的软骨细胞减少,软骨细胞簇集,基质番红花红O-固绿染色开始出现减退。4周后,软骨表面原纤维形成明显,表层及中层细胞收缩,细胞数量弥漫性增多,出现大量簇集细胞团,潮线紊乱,基质番红花红O-固绿染色明显减退,滑膜出现增生。8周后病变进一步加重,大部分软骨的表层细胞消失,软骨表面出现溃烂缺损,软骨细胞数量减少,可见坏死崩解细胞,潮线紊乱甚至消失,番红花红O-固绿染色重度减退甚至消失。滑膜亦见明显增生肥厚,出现单核淋巴细胞浸润。结论前交叉韧带切断能诱发大鼠骨关节炎的发生,其病理改变随时间增加而加重。 Objective To estahlish a rat model of osteoarthritis hy anterior cruciate ligment transaction (ACLT) and observe its pathologic features so as to provide animal model for research of osteoarthritis, Methods Forty 3-month-old Wistar rats weighing 200--250 g (including 20 males and 20 females) were enrolled in this study. In randomly selected 30 rats, one knee was subjected to ACLT (experimental group), and other knee served as blank control without surgical operation. The rats were caged and fed. The remaining 10 rats served as sham-operation group. On the postoperative day 3, 20 U penicillin was intramuscularly injected into each rat once every day to prevent infection. On the postoperative week 2 and 4, and 8, 10 rats were sacrificed in experimental and blank control groups, and 10 rats in sham-operation group killed on the postoperative week 8, Knees were harvested to observe the pathologic change of cartilage macroseopically and microscopically. Results Macroscopically, the normal luster of articular cartilage disappeared, cartilage surface was irregular on the postoperative week 2 in experi- mental group. Cartilage erosion, surface roughness and color gloomy were seen, and 4 cases out of 10 knees suffered from joint effusion and synovial hyperplasia on the postoperative week 4 week. On the postoperative week 8, cartilage surface irregulari- ties, ulcers and even cartilage defects and subchondral bone nudity could be seen, and 8 cases out of 10 knees had joint effusion and synovial hyperplasia. Microscopically, in experimental group, fibril formation and cell swelling on the cartilage surface, reduced flat cartilage cells, and cartilage cell cluster were seen, and matrix safranine O-Fast Green staining begun to subside on the postoperative week 2; OBvious fibril formation, superficial and middle cell contraction, the diffusely increased number of ceils, a large number of cell clusters, tidal lines disorders, and significantly reduced matrix safranine O-Fast Green staining could be seen, and synovia
