摘要
目的:探讨高血压非糖尿病男性患者中非酒精性脂肪性肝病(NAFLD)的患病率及其相关危险因素。方法:比较按不同体质指数(BMI)、腰臀比(WHR)分组的560例高血压非糖尿病男性患者的NAFLD患病率,并将该560例男性高血压非糖尿病患者按腹部超声检查结果分为NAFLD组(162例)和对照组(398例),观察2组间肝功能及血脂等各项指标的差异;同时应用Logistic逐步回归分析高血压患者中与NAFLD相关的危险因素。结果:高血压非糖尿男性患者中,随着BMI和WHR的升高,其NAFLD患病率明显升高,差异有统计学意义(P<0.001和P=0.02);NAFLD组中丙氨酸转氨酶(ALT)、γ-谷氨酰转移酶(γ-GT)、三酰甘油(TG)、低密度脂蛋白(LDL-C)、体重、BMI、腰围、WHR、尿酸(UA)等指标均高于对照组(P<0.05);Logistic逐步回归分析显示,BMI、ALT、LDL-C*载脂蛋B(apoB)是高血压非糖尿病男性患者合并NAFLD的独立危险因素。结论:高血压非糖尿病男性患者中,随着BMI及WHR的增高,NAFLD的患病率也随之上升;BMI、ALT、LDL-C*apoB是高血压非糖尿病男性患者合并NAFLD的独立危险因素。
Objective To investigate the prevalence and related risk factors of nonalcoholic fatty liver disease (NAFLD) in nondiabetic patients with hypertension. Methods The prevalence of NAFLD in nondiabetic hypertensive patients with different body mass index (BMI)and waist-to-hip Ratio(WHR)was compared by Chi-square test. According to abdominal ultrasound examination, 560 nondiabetic male patients with hypertension were divided into NAFLD group (162 patients) and non-NAFLD group (398 patients).Independent-samples t test was used to analyze liver function, blood lipid and other items between the two groups and logistic stepwise regression analysis was used to analyze the risk factors of NAFLD. Results The prevalence of NAFLD in nondiabetic hypertensive patients increased with the increasing of BMI and WHR (P〈0.001 and P=0.02, respectively).Alanine aminotransferase (ALT), gamma-glutamyltransferase (γ-GT), triglyceride (TG), low-density lipoprotein cholesterol (LDL-C), body weight, BMI, waist circumference, WHR, and serum uric acid in NAFLD group were significantly higher than those in the group without NAFLD. Logistic stepwise regression analysis demonstrated that BMI, ALT and LDL-C * apolipoprotein B were the independent risk factors of NAFLD. Conclusions The prevalence of NAFLD increases with the increasing of BMI and WHR in nondiabetic patients with hypertension. BMI, ALT and LDL-C * apolipoprotein B are the independent risk factors of NAFLD in nondiabetic patients with hypertension.
