Relationship between serum N-terminal pro-brain natriuretic peptide and left ventricular function in elderly patients with chronic heart failure

摘要 目的探讨老年慢性心力衰竭(CHF)患者血浆脑钠肽前体(NT-proBNP)水平与左室功能的关系及临床意义。方法102例CHF患者按美国心脏病协会(NYHA)分级方法分为心功能Ⅱ、Ⅲ、Ⅳ级3组,18名健康老年人纳入对照组。电化学发光双抗体免疫夹心法测定4组血浆NT-proBNP,二维超声测定左室舒张末期内径(LVEDD),Simpon双平面法测定左室射血分数(LVEF)。结果心衰各组NT-proBNP水平、LVEDD高于对照组,LVEF低于对照组。血浆NT-proBNP水平、LVEF、LVEDD在4组间两两比较均有统计学差异(P<0.05或P<0.01)。NT-proBNP水平与LVEDD呈正相关(r=0.76,P<0.01),而与LVEF呈负相关(r=-0.68,P<0.01)。结论NT-proBNP可以用来评估老年心衰患者的左室功能,是反映心衰的客观指标。 Objective To investigate the relationship between the level of serum Nterminal probrain natriuretic peptide(NTproBNP)and left ventricular function in elderly patients with chronic heart failure(CHF) and the clinical significance. Methods The immunofluscence double Sandwich method was used to detect NTproBNP level in 102 elderly patients with CHF,while left ventricular end diastolic diameter(LVEDD),left ventricular eject fraction(LVEF) were measured by Simpson.The patients with CHF were devided into 3 groups according to NYHA and 18 healthy elderly subjects were enrolled in control group. Results The level of serum NTproBNP and LVEDD were higher and LVEF was lower in patients with CHF than those in control group (P〈0.05 or P〈0.01).The increase of NTproBNP levels was closely correlated with the severity of NYHA functional class.NTproBNP levels were positively correlated with LVEDD(r=0.76,P〈0.01) and negatively correlated with LVEF(r=-0.68,P〈0.01). Conclusions NTproBNP is correlated with the severity of symptoms of CHF, as a reliable laboratory marker in elderly patients with CHF at present.
