摘要
目的探讨Tei指数在评价原发性高血压病患者左室收缩及舒张功能中的临床应用价值。方法原发性高血压病患者47例,平均年龄(53.2±15.6)岁,均符合WHO关于原发性高血压病的诊断标准。将全部患者分为3组:左室构型正常者为A组(17例);向心性肥厚者为B组(19例);离心性肥厚者为C组(11例)。正常对照者为D组(20例)。常规超声心动图评价参数包括测量左室舒张末期内径(LVDd)和舒张末期左室壁相对厚度(RWT),二尖瓣口血流频谱舒张早期和舒张晚期血流峰值之比(E/A),E峰减速时间(EDT),左室流出道血流频谱,左室射血分数(EF)以及Tei指数。结果A组、B组和C组的室间隔厚度较D组明显增厚,P<0.01;C组的LVDd较A组、B组和D组明显增大,P<0.01;A组、B组和C组的E/A均较D组减小,P<0.01;C组的EF值比D组明显减小,P<0.01;C组的Tei指数较D组明显延长,P<0.01;A组和B组的Tei指数比D组也明显减小,P<0.05,并且A组和B组与C组相比同样具有统计学意义。结论Tei指数评价左室的收缩及舒张功能其方法简单、准确且敏感,具有重要临床价值。
Objective To investigate the clinical applications of Tel index for evaluating global left ventriculat function in hypertensive patients.Methods Forty-seven patients with essential hypertension which was corresponded to criterion of WHO were divided into normal left ventricle (A group,17 cases), concentric thickening(B grnup,19 c.ases)and eccentric thickening (C group, 11 cases), simuhaneously,20 normal subjects were taken as control group .The conventional echo-Doppler parameters used for the e,)aluation included the diastolic diameters of left ventricle (LCDd), RWT, E/A rotio,deceleration time (DT),ejeclion fraction(EF)and Tel index,Results RWT was significantly larger in A, B and C group than in control group (P〈0.01),I.VDd was significantly larger in C grnup than in A,B and control group (P〈0.01),E/A ratio was significantly less in A,B and C group than in control group (P〈0.01),EF was significantly less in C group than in control group (P〈0.01),Tei index was significantly prolonged in C group than control group (P〈0.01),Tei index was significantly reduced in A and B group than in control group (P〈0.05),and the comparison between A and B group vs C group was also statistic difference. Conclusion Tel index is a simple, accurate and sensitive parameter for evaluating global left ventricutar function in hypertensive patients.
