摘要
目的探讨连续性血液净化（continuous blood purification,CBP）治疗重症胰腺炎（severe acute pancreatitis,SAP）的疗效与护理要点。方法使用BRAUN-BM25连续血液净化机,应用连续静脉-静脉血液滤过（continuous veno-venous hemofiltration,CVVH）技术治疗重症胰腺炎。患者23例,其中男性16例,女性7例,置换液参照南京军区总医院配方,以前稀释方式输入,流量为3000~4000ml/h,血流量150~200ml/min。结果23例患者CVVH平均持续时间为（4.6±3.8）d。23例患者中16例好转,存活率为69.6%。CVVH前APACHEII评分平均为（14.5±4.2）（12~29）分,CVVH治疗48h后平均为（9.8±3.9）分（P〈0.05）。结论CVVH配合合理的护理治疗SAP效果较好。
Objective To investigate the effectiveness and nursing methods of treating the patients with se- vere acute pancreatitis（SAP） by continuous blood purification. Methods 23 SAP patients（ male 16, female 7） were included in this research. Using BRAUN-BM25 continuous blood purification machine to treat SAP patients by contin- uous veno-venous hemofiltration （CVVH）. Formula of displacing liquid refer to that of Nanjing General Hospital of Nanjing Military Command. Displacing liquid was input before the filter at a rate of 3000 -4000 ml/h. Blood flow was 150 - 200 mL/min. Results The average treating time of 23 patients was （4.6 ±3.8 ） d. Situation of 16 patients took a turn for the better and survival rate was 69.6%. The average score of APACHEII before CVVH was 14.5 ± 4.2, 48 h after CVVH was 9.8±3.9 （ P 〈 0. 05 ）. Conclusion The effectiveness of treating SAP patients was better by CWH coordinated with reasonable nursing.
