摘要
本文应用彩色多普勒超声对30例正常足月胎儿,20例妊高征孕妇的胎儿及20例过期孕孕妇的胎儿进行心、脑、肾及脐带血流多指标全面监测,并作多元相关回归分析。结果显示:好高征及过期孕组胎儿主动脉、肾动脉及脐动脉血流速度降低,血流阻力增加,胎儿大脑中动脉血流速度增加,血流阻力降低,与正常胎儿组有统计学差别(P<0.05)。脐动脉PI与肾动脉PI和主动脉PI呈正相关。与大脑中动脉PI呈负相关;脐动脉PI与孕龄、孕妇血压呈正相关,与新生儿Apgar评分呈负相关。与胎儿NST监护比较,能更早期发现胎儿宫内缺氧状况。本文指出,彩色多普勒超声多指标全面监测,能非常敏感地反映胎儿器官异常情况的血流动力学改变,为围产医学提供新的有价值的监测方法。
The hemodynamics parameters of the fetalumbilical, renal, aortic and middle crebral artery were evaluated with color Doppler ultrasound tech- niques (CDU) in 30 normal cases of pregnancy (normal group) and 30 cases suffering from preg- nancy induced hynertension (contrast group ) and30 cases of prolonged pregnancy (contrast group). The results showed that the changes of parameters in these arteries were parallel. When the fetus was hypoxia, the flow velocity of the fetal umbilical, renal and aortic arteries decreased, and the resis- tance increased. The changes of the flow parame- ters of middle cerebral artery were inversed . Theresults indicated that a complete CDU evamination can exaluate the hemodynamics of fetus more sen- sitive and accuracy. It's a optimal method of mon- itoring fetal hemodynamics.
