乙肝病毒抗原在离体全血中衰减引起假阴性

摘要 目的探讨离体全血中的细胞成分对乙肝抗原HBsAg和HBeAg的免疫学检测影响。方法收集80人份HBV携带者血样,每人血样分为3份,一份在离体后迅速离心祛除细胞成分,另一份在离体后3h离心祛除细胞成分,还有一份在离体后12h离心祛除细胞成分。分别以夹心ELISA试验及RIA试验定性、定量检测3份血样中的HBsAg和HBeAg水平,并进行成对资料的t检验分析。结果HBsAg和HBeAg在较高浓度时,同一来源的3份血样中抗原含量无明显差异;而HBsAg和HBeAg处于较低浓度时,3份血样中抗原含量差异明显,细胞成分祛除较晚的血样中抗原水平明显较低。结论作为外源性病原体抗原,HBsAg和HBeAg因抗原代谢作用而在离体全血中持续衰减,这种衰减可能导致一些乙肝携带者的假阴性检测。 Objective To clarify the attenuation rule of HBV antigen in whole blood in vitro and the interferential effect in clinical immunology test. Methods 80 whole blood specimens were collected in HBV carrier,each specimen was distributed into three shares, cells in one were lustrated immediately by centrifuging, another were lustrated in 3 hours, and the third were lustrated in 12 hours. Concentration of HBsAg and HBeAg in three shares of the same source were measured simultaneously after 12 hours. Results In a higher level,the concentration of HBsAg and HBeAg in three shares of the same source appears no significant difference, but in a certain lower level ,the concentration of HBsAg and HBeAg in three shares of the same source appears distinct difference,antigen levels in shares in which cells lustrated later were significant lower. Conclusion As extraneous pathogenic antigen to organism,metabolizable action caused continuously attenuation of HBsAg and HBeAg in whole blood in vitro,and the attenuation might cause false negative results in some HBV carrier.
