摘要
目的:探讨膀胱全切后改良MainzⅡ尿液转流术与Bricker回肠膀胱术的疗效。方法:回顾性分析我院2005年1月～2008年1月在膀胱全切后行改良MainzⅡ尿液转流术16例(MainzⅡ组)和Bricker回肠膀胱术12例(Bricker组)的临床资料,比较两组患者术中术后一般情况、手术相关并发症及生活质量评分(QOL)。结果:28例患者均获得随访,MainzⅡ组随访时间为21个月,Bricker组随访时间为23.6个月,两组平均年龄、术中失血量、术后下床时间及随访时间差异均无统计学意义(P>0.05);MainzⅡ组手术时间、术后并发症率、门诊复诊率均低于Bricker组;MainzⅡ组生活质量评分平均1.6分(介于满意和大致满意之间),Brickcr组生活质量评分4.67分(介于不满意与苦恼之间),两组比较差异有统计学意义(P<0.05)。结论:与经典手术Bricker术相比,改良MainzⅡ尿液转流术手术时间短,创伤较小,较易操作,术后并发症率低,患者生活质量高,与正常排尿习惯几乎相同(尤其女性患者),有仿生学意义.是医患双方均易接受的术式。
Objective：To study the efficacy of radical cystectorny with modified Mainz Ⅱ pouch rectum conduit and Bricker＇s ileal conduit. Methods.. Tolally 16 cases of modified Mainz Ⅱ pouch rectum conduit and 12 cases of Brieker＇s ileal conduit treated with radical cystectomy were studied retrospectively in our hospital from January 2005 to January 2008. We comparatively analyzed the data of the two groups of patients which included the postoperative general situation, the postoperative surgery related complications, and the postoperative health related quality of life score （QOI,）. Results： All the cases were followed up. The group modified Mainz II follow up period of 21 months,and Bricker＇s group was 23. 6 months. The mean age,the volume of bleeding,the time of follow-up and the postoperative time get out-bed of both groups were not significantly different （ P〉 0.05）. The modified Mainz Ⅱ pouch rectum conduit group had lower rate of postoperative complication, the out-patlent referral rates and the lower score in the health related quality of life than Bricker group. Mean score of the former was 1. fi points （range between satisfaction and almost satisfaction）, Bricker＇s group was 4.67 poinls （between dissatisfaction and agony）. It had a significant difference （ P 〈0.05）. Conclusions：To compared with Bricker＇s ileal conduit, the mod- ified Mainz-Ⅱ pouch rectum conduit had advantage of short operation time, less trauma, simple procedure and lower postoperative complication, and higher health related quality of life. The patients were almost with normal urination habits （especially female）. It is a better surgery pattern with significance of bionics for surgeon and pa tients.
