摘要
目的:探讨经皮冠状动脉介入治疗(PCI)前后冠心病患者血清胎盘生长因子水平变化及临床意义。方法:选择110例行择期PCI的冠心病患者,分别在术前,术后第1天、3天和5天测定血清胎盘生长因子(PlGF)水平;30例冠状动脉造影正常者作为对照。随访6个月,观察术后心血管事件(MACE)发生与PlGF水平变化之间的关系。结果:冠心病患者术后第1天血清胎盘生长因子水平均较术前增高[(14.49±3.42)∶(13.06±4.05)ng/L,P<0.01],术后第3天明显升高达到峰值[(16.44±3.25)∶(13.06±4.05)ng/L,P<0.01],术后第5天下降接近正常。发生MACE患者术前和术后血清PlGF峰值水平明显高于未发生MACE患者[(15.3±4.07)∶(10.7±2.65)ng/L,P<0.05)和(18.5±4.25)∶(12.3±3.28)ng/L,P<0.01)]。结论:冠心病患者PCI前后血清PlGF水平峰值与MACE相关,PlGF水平升高可能是冠心病患者PCI后发生MACE的危险因素之一。
Objective:To explore the dynamic changes of serum placental growth factor(PIGF) during percuta neous coronary intervention(PCI) and the effect of P1GF on clinical prognosis. Method: The total of 110 patients with coronary heart disease ( CHD ), who would be treated by selective PCI, were enrolled. Serum placental growth factor was measured before PCI and 1,3 and 5 days after PCI respsctively. Another 30 healthy people were in control group . The postoperative follow up was 6 months, relationship between serum P1GF levels and incidence of cardiovascular events were observed. Result:Serum P1GF levels on the first day after PCI were elevated as compared with those before PCI ( [14.49 ± 3.42 ]vs [13.06± 4.05] ng/ L, P〈0.01), obviously peaked on the 3rd day after PCI ([16.44±3.25 ]vs [13.06±4.05] ng/L, P〈0.01) and declined on the 5th day. The paeks of serum P1GF level before and after PCI in patients with cardiovascular events were significantly higher than those patients without cardiovascular events([15. 3±4. 07]vs [10. 7±2. 65] ng/ L,P〈0. 05;[18.5±4.25]vs[12.3± 3.28 ]ng/ L, P〈0.01)]. Conclusion: The peak of serum P1GF before and after PCI is related with cardiovascular events. Serum placental growth factor elevation could be regarded as one of risk factor in predicting cardiovascular event in CHD patients undergoing PCI.
