依托咪酯对兔主动脉阻断脊髓缺血性损伤的保护作用 被引量:8

摘要 目的探讨依托咪酯对兔主动脉阻断脊髓缺血性损伤的影响。方法取日本大耳白兔24只,随机双盲均分为3组:假手术组(A组)不阻断主动脉血流;缺血再灌注组(B组)用弹性硅胶管阻断腹主动脉血流40 min;依托咪酯保护组(C组)阻断腹主动脉血流前10 min缓慢静脉注射依托咪酯乳剂0.5 mg.kg-1,续以微量输液泵持续输注0.1%依托咪酯3 mg.kg-1.h-1至松开腹主动脉时,随即阻断腹主动脉血流40 min。A、B组以同样方法静脉注射等容量0.9%氯化钠溶液作对照。监测后肢神经功能、后肢针电极肌电图(EMG)变化、脊髓组织病理学改变、脊髓匀浆丙二醛(MDA)含量和超氧化物歧化酶(SOD)活性。结果与A、C组比较,B组日本大耳白兔后肢神经功能评分、后肢EMG和脊髓组织形态学分级均有显著病理改变(P<0.01)。B组脊髓匀浆中MDA含量显著高于A组,而C组脊髓匀浆中MDA含量显著低于B组(P<0.01);与A组比较,B组脊髓匀浆中SOD活性明显减弱,而C组脊髓匀浆中SOD活性明显强于B组(P<0.01)。结论依托咪酯对主动脉阻断脊髓缺血-再灌注损伤具有良好的保护作用,其保护作用机制可能与具有较强的抗过氧化反应作用有关。 Objective To study the effect of etomidate on spinal cord injury induced by ischemia/reperfusion (I/R) . Methods Twenty four white adult Japanese rabbits were randomly assigned to 3 groups : groupA, sham-operation group ( n = 8 ) ; group B, ischemic reperfusion group; group C, isehemia-reperfusion with etomidate. Spinal cord ischemia was induced by infra-renal aortic cross-clamp for 40 minutes in groupB and group C. Etomidate 0.5 mg · kg^-1 was intravenously injected 10 min before clamping and then infused at a rate of 3 mg·kg^-1·h^-1 until unclamping in group C,0.9% saline was used as control in other groups. After surgical operation, post-operative neurological function, electromyography of rear limbs, histopathology, malondialdehyde (MDA) levels and superoxide dismutase (SOD) activities in the spinal cord were assessed for all animals. Results Compared with group A, group B showed significant I/R injury-induced changes in the neurological function scores, histopathology and eleetromyography (P 〈 0.01 ). However, groups C treated with etomidate significantly reversed these changes of all parameters (P 〈 0.01 ). Meanwhile, I/R-induced increase in MDA content observed in group B were also significantly reduced in group C (P 〈 0.01 ) , and I/R-induced decrease in the SOD activities were also significantly prevented in etomidate - treated group ( P 〈 0.01 ). Conclusion Etomidate showed potent protective effects against spinal cord I/R injury in the rabbit model, and protected loss of antioxidant activities of the spinal cord as well.
