摘要
目的:研究天津地区呼吸道分离肠杆菌科细菌中产质粒介导超广谱β-内酰胺酶(ESBLs)菌株基因分布情况。方法:收集天津市胸科医院2004年临床分离产ESBLs肠杆菌科细菌,选取携带质粒菌株,提取其质粒DNA作为模板,采用聚合酶链反应(PCR)和序列分析确定ESBLs的基因型。结果:40株接合试验阳性产ESBLs菌株中TEM型、SHV型、CTX-M型阳性率分别为50.0%、20.0%和67.5%,PCR阳性菌株中同时产2种以上酶者高达61.3%。随机抽取5株ESBLs的PCR产物进行序列分析,其中2株TEM型的序列均属TEM-1型,1株SHV型的序列属于SHV-12型,2株CTX-M型的序列分属CTX-M3、CTX-M14型。结论:天津地区呼吸道肠杆菌科细菌ESBLs基因分型以CTX-M型为主,同时产2种以上酶的菌株居多数。
Objective: To study the genotype of the clinical isolates of Enterobacteriaceae producing plasmid-mediated extended-spectrum β-lactamases (ESBLs) from respiratory tract infection in Tianjin. Methods: PCR and DNA sequencing were used to detect the genotype of ESBLs in Tianjin Chest Hospital in 2004. Results: PCR experiment showen 50.0%blaTEM, 20.0% blaSHV, 67.5%blaCTX-M-positive results for 40 clinical strains producing ESBLs. Among strains producing positive PCR results, those who produced β-1actamases with more than 2 genotypes was up to 61.3%. Five PCR products were identified as TEM-1,TEM-1,SHV-12,CTX-M3 and CTX-M14. Conclusion: CTX-M type is the predominant genotype of ESBLs in clinical Enterobacteriaceae strains from respiratory tract infections in Tianjin, and most of them produced β-lactamases with more than 2 genotypes.
