摘要
目的:通过比较带侧孔和不带侧孔指引导管行小型猪冠状动脉支架植入术,探讨带侧孔指引导管行支架植入术的安全性和有效性。方法:30只雄性西藏小型猪随机分组为带侧孔组和不带侧孔组,分别用带侧孔和不带侧孔的右Judkins指引导管行小型猪冠状动脉支架植入术,定量冠状动脉造影测量两组左右冠状动脉血管直径,有创压力监测冠状动脉内血压,实时心电监测心律失常。结果:小型猪带侧孔组和不带侧孔组的右冠状动脉直径分别为(2.51±0.12)mm和(2.54±0.11)mm(P>0.05),左前降支直径分别为(2.40±0.09)mm和(2.42±0.12)mm(P>0.05),右冠状动脉收缩压分别下降(14.9±6.5)mmHg和(30.5±9.5)mmHg(P<0.05),左冠状动脉收缩压分别下降(15.5±6.8)mmHg和(32.4±10.2)mmHg(P<0.05)。心律失常带侧孔组发生率低(P<0.05)。结论:带侧孔指引导管完成小型猪左右冠状动脉造影和支架植入术简便、安全、有效。
Objective: To assess the safety and efficacy of stenting in mini-pig coronary arteries when guiding catheter with side holes or without side holes was used. Methods: Thirty male Tibet mini-pigs were randomly divided into 2 groups, 15 in side-hole group and 15 in no-side-hole group. 6 F guiding right Judkins catheter with or without side holes was used for stenting. Quantitative coronary angiography (QCA) was performed to measure the diameter of coronary artery before stenting. Heart rates and coronary artery pressures were monitored during operation. Results: The diameter of right coronary artery in side-hole group was (2.51±0.12) mm vs (2.54±0.11) mm in the no-side-hole group (P〉0.05), and that of the left anterior descending artery in side-hole group was (2.40±0.09) mm vs (2.42±0.12) mm in the no-side-hole group (P〉0.05). The decrease in systolic blood pressure in right coronary artery was (14.9±6.5) mmHg in side-hole group vs (30.5±9.5) mmHg in the no-side-hole group (P〈0.05), and that in the systolic blood pressure in left coronary artery was (15.5±6.8) mmHg in side-hole group vs (32.4±10.2) mmHg in the no-side-hole group (P〈0.05). Less episodes of arrhythmia is found in side-hole group than in no-side-hole group (P〈0.05). Conclusion: Guiding catheter with side holes used for stenting in mini-pig was easy to use, safe and effective.
