摘要
目的观察普罗帕酮对家兔两次缺氧性心脏停搏缺氧时间及心肌超微结构的影响。方法将16只家兔随机分为实验组和对照组。参考气管夹闭窒息法制作心跳骤停模型,夹闭前实验组静脉输注普罗帕酮3mg/kg预处理,对照组静注等剂量生理盐水。记录第1次心脏停搏时间T1。(夹闭气管导管至心脏停搏)和第2次心脏停搏时间T2(第1次心脏停搏复苏成功后15min再次夹闭气管导管至心脏停搏),并在第1次气管导管夹闭后3、6min和复苏后10min时行动脉血气检测。每组各抽取两只家兔分别于第1次夹闭10min以及复苏后第2次夹闭4min时取心脏做心肌超微结构电镜检查。结果实验组第1次缺氧性心脏停搏时间为(744.8±78.1)s,较对照组的停搏时间[(629.6±41.5)s]延长(P<0.05),但两组在复苏时间无显著性差异。实验组第2次缺氧性心脏停搏时间为(439.8±74.7)s,长于对照组的(310.4±26.4)s,P<0.01。实验组心肌细胞内超微结构损害较对照组轻微。结论应用普罗帕酮预处理能够延长在体家兔缺氧性心脏停搏时间,可提高心肌缺氧耐受能力。
Objective To investigate the effects of propafenone on the time of hypoxia and ultramicroscopic structure of myocardium in the interval of cardiac arrest. Methods 16 rabbits were randomly divided into two groups, profenone preconditioning group and control group ( P group) and control group ( C gronp, same volume of N. S), The cardiac arrest model was established by trachea - clipping resulting in asphyxia. P group was preconditioned with profenone 3mg/kg and C group was preconditioned with same volume of saline before trachea clipping. T1 (the time begin from trachea clipping to cardiac arrest) and T2 ( the time from 15 minutes after successful resuscitation to 2nd cardiac arrest by trachea clipping). Artery blood gas analysis was performed at 3 and 6 minutes after trachea clipping, as well as 10 minutes after the successful resuscitation receptively. At 10 minutes after the first clipping and 4 minutes after the second clipping,left ventricular myocardium of two rabbits from each group randomly was examined under electro - microscope. Results T1 was longer in P group than that in C group( P 〈 0.05 ). There were no differences in resuscitation time between the two groups. T2 was much longer in P group than that in C group(P 〈0.01 ). The ultramicroscopic structure damage was of myocardium milder in P group than that in C group( P 〈 0.01 ). Conclusion Propafenone preconditioning could prolong the times of anoxic cardiac arrest and increase hypoxia tolerance.
