摘要
目的探讨红细胞参数(MCV/RDW)和红细胞形态学联合检测在地中海贫血诊断中的应用价值。方法采用日本SysmexKX-21自动血细胞分析仪检查3956例样本,对符合MCV↓/RDW正常和MCV↓/RDW↑但红细胞形态学呈现大小不等,小红细胞、泪滴状红细胞、靶形红细胞和畸形红细胞可见的标本进行聚合酶链反应(PCR)法复检,另外对经PCR法确诊的100例非地中海贫血者进行MCV/RDW检测,并且对MCV↓/RDW↑的标本进行红细胞形态学复检。结果MCV/RDW和红细胞形态学联合检测的地中海贫血检出率为9.2%(365/3956),准确率为96.7%(353/365),特异度为83.0%(83/100)。结论MCV/RDW及红细胞形态学联合检测对地中海贫血筛查具有较高的诊断价值,可作为简单、方便、快速、较准确的筛查方法。
Objective To explore the value of red blood cell parameters (MCV/RDW) and red blood cell morphology combined detection in thalassemia diagnosis. Methods The Japanese SysmexKX- 21 automatic blood corpuscle analyzer was used to inspect 3956 eases , in line with the MCV ↓ / RDW normal and MCV ↓ / RDW ↑ but the red blood cell morphology presents the size different , the small red blood cell, the waterdrop shape red blood cell, the target shape red blood cell and the abnormal red blood cell carries on PCR examination again, and PCR method for the diagnosis of 100 cases of nonthalassemia who carry on the MCV/RDW examination, and on the MCV ↓ / RDW ↑ specimens earries on the red blood cell morphology examination again. Results The thalassemia detection rate of the red blood cell parameters (MCV/RDW) and the red blood cell morphology union examination was 9.2% (365/3956), the accuracy rate was 96. 700 (353/365), the specificity was 83.0% (83/100). Conclusion The red blood cell parameters (MCV/ RDW) and the red blood cell morphology union examination has the high diagnosis value in thalassemia screening , which can be used as a simple, convenient, fast and more accurate screening method.
