摘要
目的:探讨护理干预对重型颅脑损伤并高血糖患者治疗转归的影响。方法:对389例重型颅脑损伤患者从入院时就进行动态监测血糖变化,根据血糖值将226例(占58.09%)分为血糖升高组和163例(占41.91%)分为血糖正常组,及时实施护理干预措施,并根据血糖情况及时调整胰岛素用量。加强营养支持、维持水、电解质平衡,对患者家属进行有针对性的健康教育。结果:血糖升高组中有148例患者(占65.49%)血糖7~14d内恢复正常,且预后与血糖正常组两组差别无统计学意义(X2=0.63P〉0.05)。有78例患者血糖持续升高,因颅脑损伤严重而死亡,病死率34.51%,明显低于文献报道的48.53%。结论:对重型颅脑损伤后高血糖患者及时动态监控血糖,及时采取护理干预措施,能降低血糖水平,减轻脑细胞损害,降低死亡率,提高救护质量。
Objective: Discussion of impact on nursing intervention on treatment of patients with severe head injury and high blood sugar. Method: Monitoring the dynamic changes in blood glucose of 389 cases with severe head injury from the hospital. Patients were divided into two groups according to the blood sugar level, 226 cases in elevated blood glucose group and 163 cases in normal blood sugar group. The patients were given the timely implementation of nursing interventions, adjustment of the amount of insulin based on the blood sugar level, and strengthen of nutritional support and maintain water and electrolyte balance, the family members of patients were given targeted health education. Result: The blood glucose level of 148 pa- tients { accounting for 65.49% ) in elevated blood sugar group was back to normal in 7 to 14 days, compared with normal blood sugar group there was no significant difference ( X2 = 0.63, p 〉 0.05 ). 78 cases with persistent high blood sugar was dead as a result of severe brain injury, and the death rate was 34. 51 percent, significantly lower than 48.53 percent reported in the literature. Conclusion: Dynamic monitoring of blood glucose of severe head injury patients with high blood sugar in time, and taking care interventions in a timely manner can reduce blood sugar levels, reduce the damage brain cells, reduce mortality and improve the quality of care.
