摘要
目的探讨脉压(PP)、脉压指数(PPI)与冠状动脉(冠脉)狭窄程度的相关性,并比较两者用以评价冠脉狭窄程度的相对优越性。方法547例行冠脉造影患者的冠心病(CHD)病变的严重程度分别用冠脉病变的血管支数、冠脉狭窄程度和冠脉病变评分表示。测定外周肱动脉收缩压(SBP)和舒张压(DBP),并计算PP及PPI。结果冠脉病变程度随增龄而逐渐加重,冠脉狭窄积分3支病变组>双支病变组>单支病变组>冠脉正常组为89.1±38.6 vs57.1±32.2 vs 26.0±22.1 vs 3.7±2.7(H=239.963,P<0.01);PP和PPI冠脉病变组>冠脉正常组;3支病变组>1支病变组,PP为(53±11)mm Hg vs(48±11)mm Hg,PPI为0.413±0.06 vs 0.375±0.06(均P<0.01);直线相关分析结果表明PP、PPI均与CHD的发生呈显著正相关(r值分别为0.547,0.632;均P<0.01)。多因素Logistic回归分析结果显示,PPI与CHD冠脉狭窄程度的关系最为密切,OR值为1.012,95%可信区间为(1.261,1.447)。结论脉压及脉压指数是较为简便的反映大动脉弹性的参数,与冠脉狭窄程度密切相关,且脉压指数在一定程度上较脉压有更大的优势。
Objective To study the relation of pulse pressure(PP) and pulse pressure index(PPI) to the severity of coronary artery stenosis. Methods Five hundreds and forty-seven patients, mean age 35-85 (58.8 ± 11.1) years old, 398 males and 149 females were included. All patients underwent coronary angiography. Brachial systolic blood pressure (SBP),diastolic blood pressure(DBP), PP and PPI were measured. The severity of coronary artery stenosis was reflected by the coronary number with stenosis,the degree of coronary stenosis and the score of coronary lesion. Results The severity of coronary artery stenosis increased with aging. There was a trend of increase of the score of coronary lesion in the non-CHD group, single-branch group, double-branch group, and multiply-branch group 3.7 ± 2.7 v s 26.0 ± 22.1 vs 57.1±32.2 vs 89.1±38.6( H =239. 963, P 〈0.01). PP and PPI were significantly different between CHD group and non-CHD group. PP and PPI in the multiply-branch group were significantly higher than those in the single- branchgroup,PP (53±11) mm Hg vs(48±11) mm Hg, PPI 0.413±0.06 vs 0.375±0.06(P 〈0.01). Linar correlation analysis indicated that PP and PPI were significantly positively associated with the occurrence of CHD( r = 0. 547,0. 632, P 〈 0.01). Logistic multivariate analysis indicated that PPI was closely related to the severity of coronary artery lesions( OR =1. 012,95% confidence interval=1. 261,1. 447). Conclusion PP and PPI are the simple parameters reflecting the elasticity of large artery and closely related to the severity of coronary artery stenosis. PPI seems better than PP in these aspects.
