摘要
目的:了解女性生殖系解脲支原体(UU)感染的临床表现;检测常用抗生素对UU的敏感性,以指导UU感染的治疗。方法:观察100例女性生殖系UU感染患者的临床症状体征及合并症情况,并取宫颈分泌物进行UU培养及药物敏感性试验。按药敏报告出来前、后开始治疗分为经验性用药组(A组,38例)和据药敏用药组(B组,62例),对照两组治疗效果。结果:10种抗生素药敏试验结果显示,强力霉素、美满霉素、交沙霉素的耐药率最低,分别为3%、8%、15%;而氧氟沙星、罗红霉素、左旋氧氟沙星的耐药率最高,分别为91%、82%、79%。对两类抗生素均耐药的占16%,对三类抗生素同时耐药的只有1%。治疗效果上,A组治愈率68.4%,B组治愈率95.2%,B组疗效显著优于A组(P<0.01)。结论:对妇科炎症患者应重视对UU感染的筛查。目前UU对喹诺酮类和大环内酯类抗生素的耐药性均明显升高,两种或三种药物联合治疗可能效果最佳。
Objective: To investigate clinical features of ureaplasma urealytieum ( UU ) infection in female genital system and detect the sensitivity of common antibiotics to UU in order to guide the treatment of UU infection. Methods:Clinical symptoms, signs and complications of 100 female patients infected UU in genital system were observed, and cervical secretions of these cases were collected for UU culture and drug sensitivity tests. They were divided randomly into empirical treatment group ( group A, n=38 ) and susceptive drug treatment group ( group B, n = 62), respectively. After the treatment, the therapeutic results were compared between these two groups. Results:Tile results of 10 kinds of antibiotic sensitivity test showed that the rate of doxycycline, minocycline and josamycin resistance was lower at 3%, 8%, 15%, respectively, while the rate of ofloxacin, roxithromycin and levofloxacin resistance was higher at 91%, 82%, 79%, respectively. The rate of duplicate antibiotic resistance was 16%, while triple antibiotic resistance was only 1%. The cure rate in group A and group B was 68.4% and 95.2% ,respectively, which demonstrated that therapeutic results in group B was significantly better than that in group A ( P 〈0.01 ). Conclusion: Now quinolones and/or macrolides resistance in UU increase significantly and combination therapy with two or three antibiotics may be a good selection of treatment of this infection in female genital system.
