膨体聚四氟乙烯假体修复隆鼻术后外形不佳 被引量:19

The clinical application of expanded polytetrafluoroethylene in the correction of failed augmentation rhinoplasty

摘要 目的探讨应用膨体聚四氟乙烯在隆鼻修复术中的手术技巧及应用价值。方法选择隆鼻术后效果不佳68例患者,手术取出原有的硅橡胶鼻假体,将膨体聚四氟乙烯加强型补片,根据患者的脸型、鼻型、要求以及医学美学观点,进行雕刻后置入体内。其中24例取自体耳软骨,固定于假体的鼻尖部位,以改观鼻尖效果。结果本组患者68例,2例术后1个月时,出现鼻根部过宽,触之假体偏硬,其中1例取出假体修剪后,再次置入,效果良好,另外1例3个月后,逐渐变软,未加处理;还有1例鼻尖部发硬,3个月后缓解;其余均取得良好的手术效果。结论膨体聚四氟乙烯为较理想的隆鼻材料替代品,尤其适合硅橡胶隆鼻术后,外观不佳、鼻尖部张力过大导致鼻尖皮肤变薄的患者;假体复合自体耳软骨能有效延长鼻长度,抬高鼻尖。 Objective To discuss the application of expanded polytetrafluoroethylene in the correction of failed augmentation rhinoplasty. Methods A series of 68 patients with unsatisfied nasal contour after augmentation rhinoplasty were employed in this study. During the surgeries, the former silicone implants were removed and then expanded polytetrafluoroethylenes sculptured according to face and nose contour, as well as patient's expecta- tion were implanted. Autogeneic ear cartilages were harvested and fixed onto the nasal tip portion of the implants in 24 patients for the improvement of nasal tip contour. Results Within these 68 patients, wide nasion and implant stiffness were observed in 2 patients. One patient was treated with removing and reshaping the implant, while the other patient was followed up for 3 months and the symptom mentioned - above disappeared. Nasal tip stiffness was observed in one patient, while the symptom disappeared after 3 months. Satisfactory results were achieved in the rest of patients. There was no immunological rejection, implant exposure or swing. Conclusion Expanded polytetrafluoroethylene implant is quite an ideal alternative for augmentation rhinoplasty, especially for the correction of failed silicone implantation with unfavorable contour and excessive nasal tip tension. Implant combined with autogeneic ear cartilage is effective for the elongation of nasal length and elevation of nasal tip.
